文拉法辛与劳拉西泮治疗抑郁症伴焦虑障碍的临床效果比较

Comparative study on the clinical effect of venlafaxine and lorazepam on the treatment of depression with anxiety disorder

目的比较文拉法辛与劳拉西泮治疗抑郁症伴焦虑障碍的临床效果。方法选取2019年3月至2020年9月河北省秦皇岛市九龙山医院收治的抑郁症伴焦虑障碍患者80例为研究对象。根据随机数字表法将患者分为对照组(40例)和观察组(40例)。对照组患者接受劳拉西泮治疗,观察组患者接受文拉法辛治疗,2组均治疗1~2个月。比较2组临床疗效、治疗前后汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)及匹兹堡睡眠质量指数(PSQI)量表评分。结果观察组总有效率高于对照组[97.5%(39/40)比82.5%(33/40)],差异有统计学意义(P<0.001)。治疗前,2组HAMD、HAMA评分及PSQI量表各项评分和总分比较差异均无统计学意义(均P>0.05)。治疗后,2组HAMD、HAMA、睡眠质量、睡眠障碍、睡眠时间、入睡时间、日间功能障碍评分及PSQI量表总分均低于治疗前,且观察组均低于对照组(均P<0.05)。结论文拉法辛可明显改善抑郁症伴焦虑障碍患者的心理状态及睡眠质量,且临床疗效优于劳拉西泮。

Objective To compare the clinical effect between venlafaxine and lorazepam on the treatment of depression with anxiety disorder.Methods Totally 80 patients with depression and anxiety disorder admitted to Qinhuangdao Jiulong Mountain Hospital,Hebei Province from March 2019 to September 2020 were enrolled.Patients were randomly divided into the control group(40 cases)and the observation group(40 cases).The control group was treated with lorazepam,the observation group was treated with venlafaxine,and both groups were treated with 1-2 months.The clinical effect,Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA),and Pittsburgh sleep quality index(PSQI)scores before and after treatment were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group[97.5%(39/40)vs 82.5%(33/40)](P<0.001).There were no significant differences in HAMD,HAMA and PSQI scores between the two groups before treatment(all P>0.05).After treatment,scores of HAMD,HAMA,sleep quality,sleep disturbance,sleep duration,sleep latency,daytime dysfunction and total PSQI score of both groups were lower than those before treatment,and the scores of the observation group were lower than those of the control group(all P<0.05).Conclusion Venlafaxine can significantly improve the psychological state and sleep quality of patients with depression and anxiety disorder,and the clinical efficacy is better than lorazepam.
