不同血管通路血液透析的应用效果观察

Application Effect Observation of Hemodialysis with Different Vascular Accesses

目的:分析不同血管通路血液透析患者的并发症发生情况以及透析充分性。方法:选取2020年1-12月重庆市奉节县人民医院收治的58例行血液透析的患者作为研究对象,随机分为观察组和对照组,各29例。对照组应用颈内静脉带隧道和涤纶套导管(TCC)通路,观察组应用自动体静脉内瘘(AVF)通路。比较两组并发症发生情况和透析效果。结果:两组尿素、总蛋白、白蛋白、血红蛋白、C反应蛋白、甘油三酯、低密度脂蛋白以及高密度脂蛋白水平比较,差异均无统计学意义(P>0.05)。两组Kt/V值比较,差异无统计学意义(P>0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:AVF和TCC在血液透析治疗过程中均可获得良好的效果,具有较理想的透析充分性,然而AVF血管通路发生并发症的概率更低,是首选的长期血管通路。

Objective:To analyze the complications and dialysis adequacy of hemodialysis patients with different vascular accesses.Methods:A total of 58 patients undergoing hemodialysis in People's Hospital of Fengjie County of Chongqing City from January 2020 to December 2020 were selected as the study subjects and randomly divided into observation group and control group with 29 cases in each group.The control group applied internal jugular vein tunneled cuffed catheter(TCC)access,and the observation group applied autogenous arteriovenous fistula(AVF)access.The complications and dialysis effects in two groups were compared.Results:There were no significant differences in urea,total protein,albumin,hemoglobin,C-reactive protein,triglycerides,low density lipoprotein and high density lipoprotein levels between the two groups(P>0.05).There were no significant differences in Kt/V values between the two groups(P>0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:In hemodialysis,both AVF and TCC can achieve good effect and ideal dialysis adequacy,but the complication rate of AVF access is lower,and it is the first choice for long-term vascular access.
