不同中药方治疗糖尿病周围神经病变的效果研究

Research on Effect of Different Chinese Prescriptions in Treatment of Diabetic Peripheral Neuropathy

目的:比较不同中药方治疗糖尿病周围神经病变(DPN)的临床疗效。方法:选取2019年10月-2020年10月灌云县中医院收治的60例DPN患者作为研究对象,根据入院顺序分为黄芪桂枝五物汤组、当归四逆汤组、补阳还五汤组,各20例。每组给予相应方剂治疗,持续治疗16周。比较三组患者治疗前后中医证候积分、视觉模拟评分法(VAS)评分、临床疗效和安全性。结果:治疗前,三组患者中医证候积分比较,差异无统计学意义(P>0.05);治疗后,三组中医证候积分均下降,且当归四逆汤组低于补阳还五汤组和黄芪桂枝五物汤组,差异有统计学意义(P<0.05)。治疗前,三组患者VAS评分比较,差异无统计学意义(P>0.05);治疗后,三组VAS评分均降低,且当归四逆汤组低于补阳还五汤组和黄芪桂枝五物汤组,差异有统计学意义(P<0.05)。当归四逆汤组治疗总有效率高于补阳还五汤组和黄芪桂枝五物汤组,差异有统计学意义(P<0.05)。三组患者均未出现严重不良反应,血常规,尿常规,肝、肾功能及心电图均未发生明显异常,未出现胃肠道不良反应。结论:当归四逆汤治疗DPN的效果优于黄芪桂枝五物汤和补阳还五汤,可更显著地提升患者生活质量,具有较高的临床价值。

Objective:To compare the clinical efficacy of different Chinese prescriptions for treating diabetic peripheral neuropathy(DPN).Methods:A total of 60 patients with DPN treated in the Guanyun County Hospital of Traditional Chinese Medicine from October 2019 to October 2020 were selected as the study subjects.According to the order of admission,they were divided into Huangqi Guizhi Wuwu Decoction group,Danggui Sini Decoction group and Buyang Huanwu Decoction group,with 20 cases in each group.Each group was given corresponding prescriptions for continuous 16 weeks.The traditional Chinese medicine syndrome integral score,visual analog scale(VAS)score,clinical efficacy and safety of the three groups were compared.Results:Before treatment,the traditional Chinese medicine syndrome integral score of the three groups were compared,and the difference was not statistically significant(P>0.05);After treatment,the traditional Chinese medicine syndrome integral score in three groups decreased,and the score in Danggui Sini Decoction group was lower than that in Buyang Huanwu Decoction group and Huangqi Guizhi Wuwu Decoction group,and the difference was statistically significant(P<0.05).Before treatment,the VAS scores of the three groups were compared,and the difference was not statistically significant(P>0.05);After treatment,the VAS scores in three groups decreased,and the score in Danggui Sini Decoction group was lower than that in Buyang Huanwu Decoction group and Huangqi Guizhi Wuwu Decoction group,and the difference was statistically significant(P<0.05).The total effective rate of treatment in Danggui Sini Decoction group was higher than that in Buyang Huanwu Decoction group and Huangqi Guizhi Wuwu Decoction group,and the difference was statistically significant(P<0.05).There were no serious adverse reactions in the three groups,no significant abnormalities in blood routine test,urine routine test,liver function,kidney function and electrocardiogram,and no gastrointestinal adverse reactions.Conclusion:Danggui Sini Decoction group is better than Buyang Huanwu Decoction group and Huangqi Guizhi Wuwu Decoction group in treatment of DPN,it can significantly improve the patient’s quality of life,and has relative higher clinical value.
