桑菊饮加减治疗小儿肺炎支原体感染后慢性咳嗽的疗效观察 被引量:15

摘要 目的观察桑菊饮加减治疗小儿肺炎支原体感染后慢性咳嗽的疗效。方法选取2018年1月—2020年4月期间南京中医药大学附属南京市中西医结合医院儿科收治的202例肺炎支原体感染后慢性咳嗽患儿为研究对象,通过随机数字表法分为治疗组101例和对照组101例。对照组给予阿奇霉素进行治疗,治疗组在对照组基础上给予桑菊饮加减进行治疗。两组患儿均治疗14 d。比较两组患儿体征改善时间,治疗后临床疗效,治疗前后体液免疫、炎症反应。结果治疗后治疗组总有效率为93.07%,明显高于对照组的75.25%,差异有统计学意义(P<0.05)。治疗组气道高反应性消失时间、啰音消失时间以及咳嗽消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗后两组患儿血清免疫球蛋白(Immunoglobulin,Ig)A、IgM、IgG,补体C3、C4水平较治疗前均升高,差异有统计学意义(P<0.05);且治疗组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患儿血清单核细胞趋化蛋白-4(Monocyte chemoattractant protein-4,MCP-4)、巨噬细胞衍生趋化因子(Macrophage derived chemokine,MDC)、白细胞介素-6(Interleukin-6,IL-6)水平较治疗前均降低,差异有统计学意义(P<0.05);且治疗组低于对照组,差异有统计学意义(P<0.05)。结论桑菊饮加减可降低肺炎支原体感染后慢性咳嗽患儿炎症反应,改善其体液免疫,加速临床体征消失,疗效确切。 Objective To investigate the effect of Modified Sangju Decoctionon chronic cough after mycoplasma pneumoniae infection in children.Methods A total of 202 children with chronic cough after mycoplasma pneumoniae infection who were admitted to the Department of Pediatrics of our hospital from January 2018 and April 2020 were classified into the observation group(n=101,azithromycin+Modified Sangju Decoction for 14 days)and the control group(n=101,azithromycin for 14 days)with the random number table method.Thetime of the improvement of physical signs,the clinical efficacy after treatment,and thehumoral immunity and inflammatory response before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 93.07%,in comparison with the 75.25%in the control group(P<0.05).Thebronchial hyperresponsiveness,rhonchus,and cough disappeared earlier in the observation groupthanin the control group(P<0.05).After treatment,the serum levels of immunoglobulin(Ig)A,IgM,and IgG,complement(C)3 and C4 were high than those beforetreatment in both groups(P<0.05)and higher in the observation group than in the control group(P<0.05).After treatment,the serum levels of monocyte chemoattractant protein-4(MCP-4),macrophage-derived chemokine(MDC),and interleukin-6(IL-6)were lower than those before treatment in both groups(P<0.05)and were lower in theobservation group than in the control group(P<0.05).Conclusion Modified Sangju Decoctioncanalleviate the inflammation in children with chronic cough after mycoplasma pneumoniae infection,improve humoral immunity,and accelerate the disappearance of clinical signs,with definite curative effect.
