悬吊固定治疗儿童移位性锁骨骨折

Suspension fixation for displaced clavicle fracture in children

目的 探讨悬吊固定治疗儿童移位性锁骨骨折的疗效。方法 采用伤肢悬吊固定治疗15例儿童移位性锁骨骨折。记录骨折愈合情况及关节活动度,采用疼痛VAS评分评估疼痛情况,采用Neer评分评估肩关节功能情况。结果 患儿均获得3个月随访。骨折愈合时间10~12周。治疗后1周疼痛VAS评分2~4(2.42±0.88)分。末次随访时,肩关节前屈175°~180°(178.41°±1.42°),后伸35°~40°(38.22°±1.77°),外展175°~180°(177.33°±1.81°),外旋55°~60°(57.89°±1.11°),内旋85°~90°(88.82°±1.02°);肩关节Neer评分92~99(96.40±1.49)分,功能恢复均为优。结论 悬吊固定治疗儿童移位性锁骨骨折操作简单,肩关节功能恢复好,疗效确切。

Objective To investigate the efficacy of suspension fixation in the treatment of displaced clavicle fractures in children.Methods A total of 15 children with displaced clavicle fractures were treated with suspension fixation of injured limbs. Fracture healing and joint range of motion were recorded,and pain situation was assessed with pain VAS, and shoulder function was assessed with Neer score.Results All children were followed up for 3months. Fracture healing time was 10 ~ 12 weeks. Pain VAS was 2 ~ 4(2. 42 ± 0. 88) points at the 1 week after treatment. At the last follow-up,the shoulder joint ROM was anterior flexion with 175° ~ 180°(178. 41° ± 1. 42°),posterior extension with 35° ~ 40°(38. 22° ± 1. 77°),abduction with 175° ~ 180°(177. 33° ± 1. 81°),the external rotation with 55° ~ 60°(57. 89° ± 1. 11°),and the internal rotation was 85° ~ 90°(88. 82° ± 1. 02°),and the shoulder Neer score was 92 ~ 99(96. 40 ± 1. 49) points. Functional recovery was excellent.Conclusions Suspension fixation is simple operation in the treatment of children′s displaced clavicle fractures, and the shoulder joint function is restored well, and the efficacy is definite.
