跟骨V形截骨保关节治疗陈旧跟骨骨折

Treatment of old calcaneal fracture with V-shaped osteotomy and joint preservation

目的 探讨跟骨V形截骨保关节治疗陈旧跟骨骨折的疗效。方法 采用跟骨V形截骨保关节治疗21例陈旧跟骨骨折患者。记录B?hler角、Gissane角、跟骨倾斜角(Pitch角)、跟骨内翻角(CVA)、跟骨高度以及AOFAS踝-后足评分。结果 患者均获得随访,时间6~32(21.4±9.1)个月。末次随访时,B?hler角、Gissane角、Pitch角、CVA角、跟骨高度及AOFAS踝-后足评分均较术前明显改善(P<0.01)。结论 跟骨V形截骨保关节是治疗陈旧跟骨骨折的有效方法,可有效改善距下关节匹配度,纠正跟骨畸形,减缓距下关节退变。

Objective To investigate the curative effect of V-shaped calcaneal osteotomy and joint preservation for treatment of old calcaneal fracture.Methods The 21 patients with old calcaneal fractures were treated by V-shaped calcaneal osteotomy and joint preservation. B?hler angle,Gissane angle,calcaneus Pitch angle,calcaneus varus angle(CVA),calcaneus height and AOFAS ankle-hindfoot score were recorded.Results All patients were followed up for 6 ~ 32(21. 4 ± 9. 1) months. At the last follow-up,B?hler angle,Gissane angle,Pitch angle,CVA,calcaneal height and AOFAS ankle-hindfoot score were significantly improved than those of the preoperation(P< 0. 01).Conclusions V-shaped calcaneal osteotomy and joint preservation are effective method in the treatment of old calcaneal fractures,which can effectively improve the matching degree of the subtalar joint,correct the calcaneal deformity and relieve the degeneration of the subtalar joint.
