“头重”考辨

A Textual Research on the "Heavy Sensation of Head"

疾病史中多详于“疾病”而略于“症状”,甚而“病”与“症”之间的概念界限也时常模糊。某些常见症既是“症状”又具备“疾病”的特征,却被边缘于疾病的典范书写不被重视。“头重”作为临床常见症散见于历代的医籍文献中却未见关于它的系统认识,欲考其源流,梳理分析其概念、病因、病机与证治,完善其理论发展脉络,以期补充中医疾病史的书写,并指导临床实践。研究内容:提炼从秦汉至明清时期“头重”演变的主要特点;分析“头重”的病因病机,总结辩证规律及其在疾病中的特殊作用;对“头重”与“头眩”进行鉴别。

In the disease history, the records are much more detailed in the "disease" rather than "symptoms", and even the conceptual boundary between "disease" and "symptom" is often blurred. Some common symptoms are "symptoms" with the characteristics of "disease", but they are marginalized and ignored by the model writing of disease. The "heavy sensation of head", a common clinical symptom that can be seen in the medical literature of previous dynasties, is rarely illustrated systematically. This paper aims to explore its origin, sort out and analyze its concept, etiology, pathogenesis and syndrome treatment, and improve its theory, so as to supplement the contents of disease history of traditional Chinese medicine and guide clinical practice. The main research results of this paper are as follows:(1) Refine the main characteristics of the evolution of "heavy and sensation of head" from Qin and Han Dynasties to Ming and Qing Dynasties;(2) Analyze the etiology and pathogenesis of "heavy and binding sensation of head" and summarize the dialectical law and its special role in the disease;(3) Identify the difference between "heavy and binding sensation of head" and "dizziness".
