摘要
目的探讨中频电疗结合康复训练治疗脑卒中偏瘫肩关节半脱位(GHS)的临床效果。方法选取2019年5月至2020年5月我院收治的82例脑卒中偏瘫GHS患者作为研究对象,采用随机数字表法将其分为研究组和对照组,各41例。研究组给予中频电疗结合康复训练治疗,对照组给予康复训练治疗。比较两组的临床效果。结果研究组的治疗总有效率高于对照组(P<0.05)。治疗后,两组的Fugl-Meyer运动功能评分法(FMA)评分高于治疗前,视觉模拟评分法(VAS)评分低于治疗前,且研究组显著优于对照组(P<0.05)。治疗后,两组的肩关节外旋、内旋和外展活动度大于治疗前,且研究组大于对照组(P<0.05)。治疗后,两组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于治疗前,且研究组显著低于对照组(P<0.05)。研究组的GHS程度分级中度占比显著低于对照组,GHS复位率高于对照组(P<0.05)。结论中频电疗与康复训练结合治疗脑卒中偏瘫GHS效果显著,可有效减轻关节疼痛,改善肩关节活动度和上肢运动功能,减轻机体炎症反应。
Objective To investigate the clinical effect of medium frequency electrotherapy combined with rehabilitation training in the treatment of glenohumeral subluxation(GHS)in stroke hemiplegia.Methods A total of 82GHS patients with stroke hemiplegia admitted in our hospital from May 2019 to May 2020 were selected as the research objects,and the patients were divided into study group and control group by random number table method,with 41 cases in each group.The study group was given medium frequency electrotherapy combined with rehabilitation training treatment,while the control group was given rehabilitation training treatment.The clinical effects of the two groups were compared.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).After treatment,the Fugl-Meyer Assessment(FMA)score of the two groups was higher than that before treatment,the Visual Analogue Scale(VAS)score was lower than that before treatment,and those in the study group were significantly better than the control group(P<0.05).After treatment,the activities of external rotation,internal rotation and abduction of shoulder joint in the two groups were greater than those before treatment,and those in the study group were greater than the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the two groups were lower than those before treatment,and those in the study group were significantly lower than the control group(P<0.05).The proportion of moderate GHS cases in the study group was significantly lower than that in the control group,and the GHS reduction rate was higher than that in the control group(P<0.05).Conclusion The medium frequency electrotherapy combined with rehabilitation training in the treatment of GHS in stroke hemiplegia has a significant effect,which can effectively reduce the joint pain,improve the activity of shoulder joint and upper limb motor function,and reduce the body’s inflammatory response.
