胰岛素泵联合个性化饮食干预对妊娠糖尿病孕妇糖脂代谢、母婴结局的影响 被引量:6

Effects of insulin pump combined with personalized diet intervention on glucose and lipid metabolism and maternal-infant outcomes in pregnant women with gestational diabetes mellitus

摘要 目的探究胰岛素泵联合个体化饮食干预对妊娠糖尿病孕妇糖脂代谢、母婴结局的影响。方法选取2018年5月至2020年5月我院接收的80例妊娠糖尿病孕妇作为研究对象,将其随机分为对照组和观察组,各40例。对照组给予胰岛素泵干预,观察组给予胰岛素泵联合个性化饮食干预。比较两组的干预效果。结果干预后,两组的空腹血糖(FBG)、糖化血红蛋白(HbA1c)及餐后2 h血糖(2 h PG)水平低于干预前,且观察组低于对照组(P<0.05)。干预后,两组的高密度脂蛋白胆固醇(HDL-C)水平高于干预前,总胆固醇(TC)及低密度脂蛋白胆固醇(LDL-C)水平低于干预前,且观察组优于对照组(P<0.05)。干预后,两组的血清晚期糖基化终产物(AGEs)、晚期氧化蛋白产物(AOPP)、血管细胞黏附分子-1(VCAM-1)水平低于干预前,且观察组低于对照组(P<0.05)。观察组的不良妊娠结局总发生率低于对照组(P<0.05)。结论胰岛素泵联合个体化饮食干预能有效改善妊娠糖尿病孕妇的糖脂代谢水平及母婴结局,降低AGEs、AOPP、VCAM-1水平,效果显著。 Objective To explore the effects of insulin pump combined with personalized diet intervention on glucose and lipid metabolism and maternal-infant outcomes in pregnant women with gestational diabetes mellitus.Methods A total of 80 pregnant women with gestational diabetes mellitus admitted in our hospital from May 2018 to May 2020 were selected as the research objects and randomly divided into control group and observation group,with 40 cases in each group.The control group was given insulin pump intervention,and the observation group was given insulin pump combined with personalized diet intervention.The intervention effects of the two groups were compared.Results After intervention,the levels of fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c)and 2 h plasma glucose(2 h PG)in the two groups were lower than those before intervention,and those in the observation group were lower than the control group(P<0.05).After intervention,the level of high-density lipoprotein cholesterol(HDL-C)in the two groups was higher than that before intervention,and the levels of total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)were lower than those before intervention,and those in the observation group were better than the control group(P<0.05).After intervention,the levels of advanced glycosylation end products(AGEs),advanced oxidation protein products(AOPP)and vascular cell adhesion molecule-1(VCAM-1)in the two groups were lower than those before intervention,and those in the observation group were lower than the control group(P<0.05).The total incidence of adverse pregnancy outcome in the observation group was lower than that in the control group(P<0.05).Conclusion Insulin pump combined with personalized diet intervention can effectively improve the glucose and lipid metabolism levels and maternal-infant outcomes in pregnant women with gestational diabetes mellitus,and reduce the levels of AGEs,AOPP and VCAM-1,with significant effect.
