芪苈强心胶囊联合美托洛尔对慢性心力衰竭患者心功能及生活质量的影响

Effects of Qili Qiangxin capsule combined with metoprolol on cardiac function and quality of life in patients with chronic heart failure

目的探讨芪苈强心胶囊联合美托洛尔对慢性心力衰竭(CHF)患者心功能及生活质量的影响。方法选择我院2019年1月至2020年12月收治的100例CHF患者作为研究对象,将其随机分为对照组和观察组,各50例。对照组采取美托洛尔治疗,观察组在对照组的基础上联合芪苈强心胶囊治疗。比较两组的心功能指标、生化指标、6 min步行距离(6-MWD)、生活质量及临床疗效。结果治疗前,两组的左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)比较,差异无统计学意义(P>0.05);治疗后,两组的LVEF高于治疗前,LVESD、LVEDD小于治疗前,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的血清金属蛋白酶组织抑制因子-4(TIMP-4)、E选择素(ES)及游离脂肪酸(FFA)水平比较,差异无统计学意义(P>0.05);治疗后,两组的血清TIMP-4水平高于治疗前,ES及FFA水平低于治疗前,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的6-MWD和明尼苏达心力衰竭生活质量问卷(MLHFQ)各维度评分比较,差异无统计学意义(P>0.05);治疗后,两组的6-MWD长于治疗前,MLHFQ各维度评分低于治疗前,且观察组优于对照组,差异具有统计学意义(P<0.05)。观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。结论芪苈强心胶囊联合美托洛尔治疗CHF的临床效果显著,有利于提升患者的心功能,改善预后,提高生存质量。

Objective To explore the effects of Qili Qiangxin capsule combined with metoprolol on cardiac function and quality of life in patients with chronic heart failure(CHF).Methods A total of 100 patients with CHF admitted in our hospital from January 2019 to December 2020 were selected as the research objects and randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with metoprolol,and the observation group was treated with Qili Qiangxin capsule on the basis of the control group.Cardiac function indexes,biochemical indexes,6 min walking distance(6-MWD),quality of life and clinical efficacy were compared between the two groups.Results Before treatment,there were no significant differences in left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)between the two groups(P>0.05);after treatment,the LVEF of the two groups was higher than that before treatment,the LVESD and LVEDD were less than those before treatment,those of the observation group were better than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in serum serum tissue inhibitors of metallproteinase-4(TIMP-4),E-selectin(ES)and free fatty acid(FFA)levels between the two groups(P>0.05);after treatment,the level of serum TIMP-4 in the two groups was higher than that before treatment,the levels of ES and FFA were lower than those before treatment,those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in 6-MWD and scores of all dimensions of Minnesota Living with Heart Failure Questionnaire(MLHFQ)between the two groups(P>0.05);after treatment,the 6-MWD of the two groups was longer than that before treatment,and the scores of all dimensions of MLHFQ were lower than those before treatment,those of the observation group were better than the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Qili Qiangxin capsule combined with metoprolol in the treatment of CHF has a significant clinical effect,it is beneficial to improve the cardiac function of patients,ameliorate the prognosis and increase the quality of life.
