摘要
目的检测Fos样抗原1(Fosl-1)、β-连环蛋白(β-catenin)及Wnt蛋白在儿童肾母细胞瘤中的表达情况,结合临床资料,探讨其与儿童肾母细胞瘤发生、转移及预后的关系。方法采用免疫组化法检测57例肾母细胞瘤及癌旁组织中Fosl-1、β-catenin及Wnt蛋白的表达。分析三种蛋白与患者临床资料的关系。采用Cox多因素回归分析探讨它们的表达与患者预后的关系。结果Fosl-1、β-catenin及Wnt蛋白在癌组织中的高表达、异常表达率均高于癌旁组织(P<0.05)。在TNM分期、转移情况等临床特征下,Fosl-1高表达与Wnt蛋白异常表达在癌组织、癌旁组织中均有差异(P<0.05);在肿瘤转移情况下,β-catenin异常表达在癌旁组织中有差异(P<0.05)。Cox多因素回归分析示转移情况、β-catenin异常表达对肾母细胞瘤患儿预后有独立影响(P<0.05)。生存曲线示Fosl-1高表达、β-catenin及Wnt异常表达均降低肾母细胞瘤患儿生存率。结论Fosl-1、β-catenin及Wnt蛋白可能参与肾母细胞瘤的发生转移,并影响其预后,为肾母细胞瘤的诊治提供新的思路及理论基础。
Objective To investigate the expression of Fosl-1,β-catenin and Wnt protein in pediatric nephroblastoma,and to explore the relationship between Fosl-1,β-catenin and Wnt protein and the occurrence,metastasis and prognosis of pediatric nephroblastoma.Methods The expression of Fosl-1,β-catenin and Wnt proteins in 57 cases of nephroblastoma and its adjacent tissues were detected by immunohistochemistry.The relationship between the three proteins and clinical data of the patients were analyzed.Cox multivariate regression analysis was used to investigate the relationship between their expression and prognosis.Results The positive rates of Fosl-1,β-catenin and Wnt protein in cancer tissues were higher than those in adjacent tissues(P<0.05).In TNM stage,metastasis and other clinical characteristics,Fosl-1 and Wnt protein abnormal expression in cancer tissues and adjacent tissues were different(P<0.05).In the case of tumor metastasis,the abnormal expression ofβ-catenin was different in the adjacent tissues(P<0.05).Coxmultivariate regression analysis showed that metastasis status and abnormal expressionofβ-catenin had independent influence on prognosis of pediatric nephroblastoma(P<0.05).The survival curveshowed that Fosl-1,β-catenin and Wnt abnormal expression were reduced the survival rate of pediatric nephroblasto-ma.ConclusionFosl-1,β-catenin and Wnt proteins may be involved in metastasis of nephroblastoma and affect itsprognosis with providing the new ideas and theoretical basis for diagnosis and treatment of pediatricnephroblastoma.
