摘要
目的:探讨布地奈德吸入联合高流量氧疗治疗重症肺炎的临床效果。方法:选取2020年1月—2021年12月于我院接受治疗的76例重症肺炎患者作为研究对象,根据治疗方案不同将其分为观察组(n=38)和对照组(n=38)。对照组接受高流量氧疗治疗,观察组接受布地奈德吸入联合高流量氧疗治疗,比较两组超氧化物歧化酶(SOD)、胰岛素样生长因子Ⅱ(IGF-Ⅱ)以及白细胞介素-8(IL-8)水平,急性生理与慢性健康评分(APACHEⅡ)与序贯器官衰竭评分(SOFA),第1s最大呼气量(FEV1)、最大呼气峰流量(PEF)、第1s最大呼气量/用力肺活量(FEV1/FVC),血氧饱和度、血氧分压以及氧合指数。结果:观察组SOD水平高于对照组、IGF-Ⅱ和IL-8水平低于对照组(P<0.05);观察组APACHⅡ评分、SOFA评分低于对照组(P<0.05);观察组FEV1、PEF、FEV1/FVC高于对照组(P<0.05);观察组血氧饱和度、血氧分压以及氧合指数高于对照组(P<0.05)。结论:布地奈德吸入联合高流量氧疗治疗重症肺炎患者的效果确切,可提升患者健康水平,降低器官损伤程度,改善肺功能以及血气指标。
Objective:To investigate the clinical effect of budesonide inhalation combined with high-flow oxygen therapy in the treatment of severe pneumonia.Methods:A total of 76 patients with severe pneumonia treated in our hospital from January 2020 to December 2021 were selected and divided into observation group(n=38)and control group(n=38)according to different treatment regimens.The control group was treated with high flow oxygen therapy,and the observation group was treated with budesonide inhalation combined with high flow oxygen therapy.The levels of superoxide dismutase(SOD),insulin-like growth factorⅡ(IGF-Ⅱ)and interleukin-8(IL-8),acute physiology and chronic health evaluationⅡ(APACHEⅡ)and sequential organ failure score(SOFA),maximum expiratory volume in one second(FEV1),maximum peak expiratory flow(PEF),maximum expiratory volume in one second/forced vital capacity(FEV1/FVC),blood oxygen saturation,blood oxygen partial pressure and oxygenation index were compared between the two groups.Results:The SOD level in the observation group was higher than that in the control group,and the levels of IGF-Ⅱand IL-8 in the observation group were lower than those in the control group(P<0.05).FEV1/FVC was higher than the control group(P<0.05).Blood oxygen saturation,blood oxygen partial pressure and oxygenation index in the observation group were higher than those in the control group(P<0.05).Conclusion:The effect of budesonide inhalation combined with high-flow oxygen therapy in the treatment of patients with severe pneumonia is exact,which can improve the health level of patients,reduce the degree of organ damage,and improve lung function and blood gas indicators.
