Rotarex系统在TASCⅡC/D级主髂动脉闭塞的应用分析

Analysis of Rotarex mechanical thrombectomy in the treatment of TASCⅡC/D aortoiliac occlusive disease

目的探讨Rotarex血栓清除系统在TASCⅡC/D级主髂动脉闭塞中的应用疗效。方法回顾性分析2017年5月至2021年3月在佛山市第一人民医院采取Rotarex血栓清除系统联合血管腔内成形治疗的35例TASCⅡC/D级主髂动脉闭塞患者的临床资料和治疗结果。结果患者均成功完成Rotarex机械吸栓治疗,技术成功率100%。患者术后3 d的ABI较术前明显增高[(1.09±0.23)vs(0.28±0.09)],差异有统计学意义(t=10.3,P<0.001)。患者均完成随访,平均随访(13.1±2.5)个月,术后12个月的一期通畅率为94.3%(33/35)。围术期和随访期间均无出血、髂动脉破裂、死亡等严重并发症发生。结论Rotarex系统可对TASCⅡC/D级主髂动脉闭塞的血栓性病变进行有效减容,联合血管腔内成形治疗的疗效满意。

Objective To evaluate the efficacy of Rotarex mechanical thrombectomy in the treatment of TASCⅡC/D aortoiliac occlusive disease.Methods The clinical data and treatment outcomes of 35 patients with TASCⅡC/D aortoiliac occlusive disease who treated with Rotarex thrombectomy system in the First People's Hospital of Foshan From May 2017 to March 2021 were analyzed retrospectively.Results All patients completed Rotarex mechanical thrombectomy successfully,and the technical success rate was 100%.The ABI of patients three days after operation was significantly higher than that before operation[(1.09±0.23)vs(0.28±0.09)],and the difference was statistically significant(t=10.3,P<0.001).All patients completed follow-up,with an average time of(13.1±2.5)months.The primary patency rate at 12 months after surgery was 94.3%(33/35).There were no serious complications such as bleeding,iliac artery rupture and death during the perioperative and follow-up period.Conclusion Rotarex thrombectomy system can effectively reduce the volume of thrombotic lesions of TASCⅡC/D aortoiliac occlusive disease.Its curative effect is satisfactory when combined with endovascular angioplasty.
