长鞘+Navien导管介入术与短鞘+导引导管治疗颅内动脉瘤的效果比较

摘要 目的:探讨长鞘+Navien导管介入术与短鞘+导引导管治疗颅内动脉瘤的效果。方法:选取2017年1月-2020年12月萍乡市人民医院神经外科接收的80例颅内动脉瘤患者,按照治疗方式将其分为对照组与观察组,每组40例。对照组采用短鞘+导引导管行颅内动脉瘤介入治疗;观察组采用长鞘+Navien导管介入治疗。比较两组手术时间、住院时间、手术并发症、术后动脉瘤的完全栓塞率、残余率、复发率及神经功能缺损评分(NIHSS)。结果:观察组手术时间及住院时间均短于对照组(P<0.05)。两组术中血管痉挛、术中动脉瘤破裂、术后缺血性并发症、术后出血性并发症发生率比较,差异均无统计学意义(P>0.05)。两组动脉瘤完全栓塞率、动脉瘤残留率及动脉瘤复发率比较,差异均无统计学意义(P>0.05)。术后3、6个月,两组NIHSS评分均低于术前,差异均有统计学意义(P<0.05)。术后3、6个月,观察组NIHSS评分均低于对照组,差异均有统计学意义(P<0.05)。结论:长鞘+Navien导管介入术与鞘管+导引导管颅内动脉介入栓塞术比较,可缩短手术时间及住院时间,降低术后神经功能缺损程度评分。 Objective:To explore the effect of long sheath+Navien catheter intervention and short sheath+guiding catheter in the treatment of intracranial aneurysms.Method:A total of 80 patients with intracranial aneurysms admitted to the Department of Neurosurgery of Pingxiang People’s Hospital from January 2017 to December 2020 were selected,they were divided into control group and observation group according to treatment method,with 40 cases in each group.The control group was treated with short sheath+guiding catheter,the observation group was treated with long sheath+Navien catheter intervention.The operation time,hospital stay,surgical complications,complete embolization rate,residual rate,recurrence rate and neurological deficit scores(NIHSS)were compared between the two groups.Result:The operation time and hospital stay in the observation group were lower than those in the control group(P<0.05).There were no significant differences in the rates of intraoperative vasospasm,intraoperative aneurysm rupture,postoperative ischemic complications and postoperative hemorrhagic complications between the two groups(P>0.05).There were no significant differences in the complete embolization rate,residual rate and recurrence rate of aneurysms between the two groups(P>0.05).At 3 and 6 months after operation,the NIHSS scores in both groups were lower than those before operation,the differences were statistically significant(P<0.05).At 3 and 6 months after operation,the NIHSS scores in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Long sheath+Navien catheter intracranial artery interventional embolization compared with short sheath+guiding catheter,it can shorten the operation time and hospital stay,and reduce the score of postoperative neurological deficit.
