关节镜下肩袖修复术在肩袖损伤中的疗效及对结构参数、肩关节功能的影响研究

Study on the Effect of Arthroscopic Rotator Cuff Repair on Rotator Cuff Injury and Its Influence on Structural Parameters and Shoulder Joint Function

目的:探究及观察关节镜下肩袖修复术在肩袖损伤中的疗效及对结构参数、肩关节功能的影响。方法:将2019年5月-2021年12月福建医科大学附属泉州第一医院的82例肩袖损伤患者根据随机的原则分为对照组和观察组,每组41例。对照组进行开放式肩袖修复术治疗,观察组则进行关节镜下肩袖修复术治疗。比较两组的肩袖损伤治疗总有效率、治疗前后的疼痛情况[视觉模拟评分法(VAS)评分]、结构参数[肩峰-肱骨头间距(A-H)及肱骨头上移系数(UMI)]、肩关节功能[美国加州大学洛杉矶分校(UCLA)肩关节评分]及肩主动活动范围。结果:观察组的肩袖损伤治疗总有效率显著高于对照组,差异有统计学意义(P<0.05);治疗前两组的VAS评分、结构参数、UCLA评分及肩主动活动范围比较,差异均无统计学意义(P>0.05),治疗后1、4及8周观察组的VAS评分均显著低于对照组,治疗后1个月及3个月观察组的结构参数、UCLA评分及肩主动活动范围均显著优于对照组,差异均有统计学意义(P<0.05)。结论:关节镜下肩袖修复术在肩袖损伤患者中的疗效较好,且对结构参数及肩关节功能的改善作用更为突出,在肩袖损伤患者中的应用价值相对更高。

Objective:To investigate and observe the effect of arthroscopic rotator cuff repair on rotator cuff injury and its influence on structural parameters and shoulder joint function.Method:A total of 82 patients with rotator cuff injury in Quanzhou First Hospital Affiliated to Fujian Medical University from May 2019 to December 2021 were divided into control group and observation group by the random principle,with 41 cases in each group.The control group was treated with open rotator cuff repair,the observation group was treated with arthroscopic rotator cuff repair.Then the total effective rates of rotator cuff injury,pain situation[visual analogue scale(VAS)score],structural parameters[acromoin-humeral head distance(A-H)and upward migration index(UMI)],shoulder joint function[University of California,Los Angeles(UCLA)score]and range of active shoulder movement before and after the treatment of two groups were compared.Result:The total effective rates of rotator cuff injury of observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05);the VAS scores,structural parameters,UCLA scores and range of active shoulder movement of the two groups before the treatment were compared,the differences were not statistically significant(P>0.05),the VAS scores of observation group at 1,4 and 8 weeks after treatment were all significantly lower than those of control group,the structural parameters,UCLA scores and range of active shoulder movement of observation group at 1 and 3 months after treatment were all significantly better than those of control group,the differences were statistically significant(P<0.05).Conclusion:The effect of arthroscopic rotator cuff repair in the patients with rotator cuff injury is better,and its improvement role for structural parameters and shoulder joint function is more prominent,so its application value in the patients with rotator cuff injury is relatively higher.
