自动弹力线套扎术联合中药坐浴治疗混合痔的临床研究

Application of Automatic Elastic Thread Ligation Combined with Chinese Herbal Bath in the Treatment of Mixed Hemorrhoids

目的:探讨自动弹力线套扎术联合中药坐浴治疗混合痔患者的临床效果。方法:选取2020年1月-2021年12月在萍乡市人民医院肛肠外科手术治疗的混合痔患者共90例,手术均采用自动弹力线套扎术,将其随机分为A、B两组,每组45例,A组采用术后中药坐浴,B组采用术后高锰酸钾外用片坐浴;观察两组的术后恢复、患者满意度、复发等情况。结果:A组的创面愈合时间[(14.53±2.27)d]明显短于B组[(15.73±2.87)d](P<0.05);A组术后第7天的疼痛评分明显低于B组(P<0.05);A组术后第7天的伤口出血程度明显优于B组(P<0.05);A组术后第3、7天的伤口水肿程度均明显优于B组(P<0.05);A组患者满意度明显优于B组,差异有统计学意义(P<0.05);A组术后3个月复发率明显低于B组(P<0.05),但术后6个月两组的远期复发率差异无统计学意义(P>0.05)。结论:自动弹力线套扎术联合中药坐浴治疗混合痔患者效果稳定,可促进创面愈合,减少术后并发症,值得推广。

Objective:To explore the application of automatic elastic thread ligation combined with Chinese herbal bath in the treatment of mixed hemorrhoids.Method:A total of 90 patients with mixed hemorrhoids who were treated in Anorectal Surgical Department of Pingxiang People’s Hospital from January 2020 to December 2021 were selected.Automatic elastic band ligation was used for the operation.They were randomly divided into two groups,group A and group B,with 45 cases in each group.Group A was given postoperative traditional Chinese medicine sitz bath,and group B was given postoperative Potassium Permanganate Topical Tablets sitz bath;postoperative recovery,satisfaction of patient,recurrence,etc were observed in the two groups.Result:The wound healing time in group A[(14.53±2.27)d]was significantly shorter than that in group B[(15.73±2.87)d](P<0.05);the pain score of group A on the 7th day after operation was significantly lower than that of group B(P<0.05);the degree of wound bleeding in group A was significantly better than that in group B on the 7th day after operation(P<0.05);the degree of wound edema in group A were significantly better than those in group B on the 3rd and 7th day after operation(P<0.05);the satisfaction of patients in group A was significantly better than that in group B(P<0.05);the recurrence rate 3 months after operation in group A was significantly lower than that in group B(P<0.05),but there was no significant difference in the long-term recurrence rate 6 months after operation between the two groups(P>0.05).Conclusion:The automatic elastic thread ligation combined with Chinese herbal bath has stable curative effect in the treatment of mixed hemorrhoids,which can promote wound healing and reduce postoperative complications.It is worth popularizing.
