高分子俯卧位头面保护垫在椎管扩大成形术的应用研究

Study on the Application of Polymer Head Protection Pad in Prone Position in Laminoplasty

目的:探讨高分子俯卧位头面保护垫在椎管扩大成形术的应用效果。方法:选取2019年11月-2021年11月于赣南医学院第一附属医院行颈后路椎管扩大成形术患者60例,按随机数字表法分为两组,各30例。对照组使用常规头架,观察组使用高分子俯卧位头面保护垫,两组均持续使用至手术结束。比较两组手术体位摆放时间、平均手术时间、皮肤受压情况、舒适度和患者满意度。结果:观察组手术体位摆放时间、平均手术时间均短于对照组,差异均有统计学意义(P<0.05);观察组皮肤受压总发生率为6.67%,低于对照组的26.67%,差异有统计学意义(P<0.05);两组术前Kolcaba的舒适状况量表(GCQ)评分比较,差异无统计学意义(P>0.05),观察组术后GCQ中生理、心理、环境、社会文化等维度评分和患者满意度中安全性、舒适度、便捷性和稳定性等项目评分均高于对照组,差异均有统计学意义(P<0.05)。结论:高分子俯卧位头面保护垫能够缩短椎管扩大成形术患者体位摆放时间,预防皮肤损伤,可提高患者舒适度,节省手术时间,从而提升患者满意度。

Objective:To investigate the application effect of polymer head protective pad in prone position in laminoplasty.Method:A total of 60 patients who underwent posterior cervical laminoplasty in the First Affiliated Hospital of Gannan Medical College from November 2019 to November 2021 were randomly divided into two groups,with 30 cases in each group.The control group used the conventional head frame and the observation group used the polymer prone head protective pad.Both groups continued to use it until the end of the operation.The operation position placement time,average operation time,skin compression,comfort and satisfaction of patient were compared between the two groups.Result:The operation position placement time and the average operation time in the observation group were shorter than those in the control group(P<0.05);the total incidence of skin compression in the observation group was 6.67%,which was lower than 26.67%in the control group(P<0.05);there was no significant difference in preoperative Kolcaba general comfort questionnaire(GCQ)score between the two groups(P>0.05),the scores of physiology,psychology,environment and social culture in GCQ,the scores of safety,comfort,convenience and stability in patients’satisfaction in the observation group after operation were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The polymer head protective pad in prone position can shorten the placement time of patients in laminoplasty,prevent skin injury,improve patients’comfort,save operation time,and improve patients’satisfaction.
