宣肺止咳丸联合穴位注射治疗咳嗽变异性哮喘的效果及对气道炎症反应的影响

Effect of Xuanfei Zhike Pills Combined with Acupoint Injection on Cough Variant Asthma and Airway Inflammation

目的:探讨宣肺止咳丸联合穴位注射对咳嗽变异性哮喘患者的治疗效果及气道炎症反应的影响。方法:选取2020年1月-2021年12月佳木斯市中医医院收治116例咳嗽变异性哮喘患者为研究对象,随机分为观察组、对照组,每组58例。对照组以糖皮质激素穴位注射治疗,观察组以宣肺止咳丸联合糖皮质激素穴位注射治疗。治疗3个月后综合评估两组患者的临床疗效、中医症候、肺通气指标、炎症因子、治疗安全性指标。结果:治疗3个月后观察组总有效率为91.38%(53/58),高于对照组的77.59%(45/58),差异有统计学意义(P<0.05)。治疗结束后1 d观察组的中医症候积分包括呼吸急促、胸膈满闷、喉中哮鸣、咯吐不利等评分均低于对照组(P<0.05)。治疗结束后1 d观察组的肺通气指标用力肺活量(FVC)、一秒率(FEV1/FVC)、最大呼气流速(PEF)均高于对照组(P<0.05)。治疗结束后1 d观察组的炎症因子,包括趋化因子1(CXCL1)、过氧化物酶体增殖物激活受体α(PPAR-α)、钙黏蛋白(E-Ca)水平均低于对照组(P<0.05)。治疗期间观察组、对照组不良反应发生率分别为13.79%(8/58)、10.34%(6/58),两组比较差异无统计学意义(P>0.05)。结论:宣肺止咳丸联合穴位注射对咳嗽变异性哮喘患者有良好的治疗效果,能够降低患者的主要中医症候程度,改善肺通气功能,下调CXCL1、PPAR-α、E-Ca等炎症因子的表达,治疗安全性较好,可临床推荐。

Objective:To discuss the therapeutic effect of Xuanfei Zhike Pills combined with acupoint injection on patients with cough variant asthma and the effect of airway inflammation.Method:A total of 116 patients with cough variant asthma treated in Jiamusi Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected as the research objects,and randomly divided into observation group and control group,with 58 cases in each group.The control group was treated with glucocorticoid point injection,while the observation group was treated with Xuanfei Zhike Pills combined with glucocorticoid point injection.After 3 months of treatment,the clinical efficacy,TCM symptoms,pulmonary ventilation indicators,inflammatory factors and treatment safety indicators of the two groups were comprehensively evaluated.Result:After 3 months of treatment,the total effective rate in the observation group was 91.38%(53/58),which was higher than 77.59%(45/58)in the control group,with a statistically significant difference(P<0.05).1 d after the end of treatment,the scores of TCM symptoms in the observation group,including shortness of breath,fullness of chest and diaphragm,wheezing in the throat,and dyspepsia were lower than those in the control group(P<0.05).1 d after the end of treatment,the forced vital capacity(FVC),forced expiratory volume in one second/forced vital capacity(FEV1/FVC),and maximum expiratory flow rate(PEF)in the observation group were higher than those in the control group(P<0.05).1 d after the end of treatment,the inflammatory factors in the observation group including chemokine 1(CXCL1)and peroxisome proliferator activated receptor-α(PPAR-α),E-cadherin(E-Ca)in the observation group were lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group and the control group was 13.79%(8/58)and 10.34%(6/58),with no significant difference(P>0.05).Conclusion:Xuanfei Zhike Pills combined with acupoint injection has a good therapeutic effect on patients with cough variant asthma.It can reduce the degree of main TCM symptoms,improve the pulmonary ventilation function of patients and reduce expression of inflammatory factors such as CXCL1,PPAR-αand E-Ca,it is safe and can be recommended clinically.
