血清D-二聚体水平与急性主动脉夹层分型的相关性研究

Correlation between Serum D-dimer Level and Classification of Acute Aortic Dissection

目的:研究急性主动脉夹层(AAD)患者血清D-二聚体(D-D)水平与其分型及预后的相关性。方法:选取2020年1月-2021年12月河西学院附属张掖人民医院收治的90例AAD患者,按Stanford分型将患者分为A组和B组。Stanford A型为A组(n=48),Stanford B型为B组(n=42)。比较两组血清D-D水平和预后。分析ADD分型与血清D-D水平的相关性。结果:两组入院前及入院后1、12 h,A组血清D-D水平均高于B组,差异均有统计学意义(P<0.05)。A组入院24 h死亡率为39.6%,高于B组的19.0%,差异有统计学意义(P<0.05)。血清D-D水平与AAD严重程度呈正相关(P<0.01)。结论:Stanford A型急性主动脉夹层患者血清D-D水平相较于Stanford B型明显升高,且血清D-D水平越高提示AAD患者病情越严重,死亡风险越高。

Objective:To study the correlation between serum D-dimer(D-D)level and its classification and prognosis in patients with acute aortic dissection(AAD).Method:A total of 90 patients with AAD admitted to Zhang Ye People’s Hospital Affiliated to Hexi University from January 2020 to December 2021 were selected,they were divided into group A and group B according to Stanford classification.Stanford type A was group A(n=48),and Stanford type B was group B(n=42).The serum D-D level and prognosis were compared between the two groups.The correlation between ADD typing and serum D-D level was analyzed.Result:The serum D-D level in group A were higher than that in group B before admission and at 1 and 12 h after admission,the differences were statistically significant(P<0.05).The 24 h mortality rate of group A was 39.6%,which was higher than 19.0%of group B,the difference was statistically significant(P<0.05).Serum D-D level was positively correlated with the severity of AAD typing(P<0.01).Conclusion:Compared with Stanford type B,the serum D-D level of Stanford type A patients with acute aortic dissection is significantly higher,and the higher the serum D-D level,the more serious the AAD patients are,the higher the risk of death.
