热敏灸治疗膝骨性关节炎的等速肌力变化研究

摘要 目的:观察热敏灸治疗膝骨性关节炎的等速肌力变化。方法:选取2020年1月-2021年6月在江西中医药大学附属医院接受治疗的40例膝骨性关节炎患者,按随机数字表法将其分为观察组和对照组,各20例。对照组给予常规艾灸治疗,观察组给予热敏灸治疗。运用等速肌力测试系统分别于治疗前后进行等速肌力测试分析,比较两组峰力矩(PT)、腘绳肌/股四头肌(H/Q)、总做功(TW)。结果:两组治疗后伸肌、屈肌PT与TW均较治疗前提高(P<0.05);观察组治疗后伸肌、屈肌PT与TW均高于对照组(P<0.05)。观察组治疗前后H/Q比较,差异无统计学意义(P>0.05);对照组治疗后H/Q较治疗前增加(P<0.05);治疗后,观察组H/Q低于对照组(P<0.05)。结论:与常规艾灸治疗比较,热敏灸治疗膝骨性关节炎的等速肌力变化更佳,可以利用等速肌力测试的方法从生物力学改变的方向来研究热敏灸疗效机制。 Objective:To observe the isometric muscle strength changes in osteoarthritis of the knee treated with thermosensitive moxibustion.Method:A total of 40 patients with osteoarthritis of the knee who were treated in the Affiliated Hospital of Jiangxi University of Chinese Medicine from January 2020 to June 2021 were selected.They were divided into the control group and the study group according to the number randomly method,with 20 cases in each group.The control group was treated with conventional moxibustion,while the observation group was treated with thermosensitive moxibustion.Isokinetic muscle strength test was performed before and after treatment using isokinetic muscle strength test system.Peak torque(PT),hamstring/quadriceps(H/Q)and total work(TW)were compared between the two groups.Result:After treatment,PT and TW of extensor and flexor muscles in the two groups were increased compared with those before treatment(P<0.05).After treatment,PT and TW of extensor and flexor muscles in the observation group were higher than those in the control group(P<0.05).There was no significant difference in H/Q before and after treatment in the observation group(P>0.05).The H/Q of the control group was increased after treatment compared with that before treatment(P<0.05).After treatment,the H/Q of the observation group was lower than that of the control group(P<0.05).Conclusion:Compared with conventional moxibustion treatment,thermosensitive moxibustion has better isokinetic muscle strength change in the treatment of knee osteoarthritis.The method of isokinetic muscle strength test can be used to study the effect mechanism of heat sensitive moxibustion from the direction of biomechanical change.
