可弯曲支气管镜和硬质支气管镜联合介入治疗良恶性复杂性气道狭窄的临床效果

Clinical Effect of Flexible Bronchoscope Combined with Rigid Bronchoscope in the Interventional Treatment of Benign and Malignant Complex Airway Stenosis

目的:研究良恶性复杂性气道狭窄行介入治疗中应用可弯曲支气管镜联合硬质支气管镜的效果。方法:选取临沂市中心医院2018年4月-2021年4月收治的90例良恶性复杂性气道狭窄患者,依据随机数字表法,分为对照组(45例)与观察组(45例),均行介入治疗,分别采用可弯曲支气管镜、可弯曲联合硬质支气管镜,评估并比较两组临床疗效,同时对治疗前后舒适度、生活质量、气促指数及狭窄程度进行比较。结果:治疗后3个月,观察组治疗总有效率为95.56%,高于对照组75.56%,差异有统计学意义(χ^(2)=7.282,P<0.05);与对照组比较,观察组治疗后1个月狭窄程度、气促指数均更低(t=12.336、19.179,P<0.05);观察组治疗后7 d舒适度评分较对照组更高(P<0.05);观察组治疗后3个月KPS评分较对照组更高(P<0.05)。结论:良恶性复杂性气道狭窄行介入治疗中应用可弯曲支气管镜联合硬质支气管镜,可促进患者气促症状改善,降低气道狭窄程度,有利于提高舒适度,促进生活质量提高。

Objective:To study the effect of flexible bronchoscope combined with rigid bronchoscope in interventional treatment of benign and malignant complex airway stenosis.Method:A total of 90 patients with benign and malignant complex airway stenosis who were admitted to Linyi Central Hospital from April 2018 to April 2021 were selected and divided into control group(45 cases)and observation group(45 cases)according to the random number table method.Both underwent interventional therapy,using flexible bronchoscopy and flexible combined with rigid bronchoscopy,respectively,the clinical efficacy of the two groups was evaluated and compared,and the comfort,quality of life,breathlessness index and stenosis before and after treatment were compared.Result:Three months after treatment,the total effective rate of the observation group was 95.56%,which was higher than 75.56%of the control group,the difference was statistically significant(χ^(2)=7.282,P<0.05).Compared with the control group,the monthly stenosis degree and breathlessness index in the observation group were lower(t=12.336,19.179,P<0.05);the comfort score of the observation group 7 days after treatment was higher than that of the control group(P<0.05);the KPS of the observation group 3 months after treatment was higher than that of the control group(P<0.05).Conclusion:The application of flexible bronchoscope combined with rigid bronchoscope in interventional treatment of benign and malignant complex airway stenosis can improve the symptoms of shortness of breath,reduce the degree of airway stenosis,improve comfort,and improve quality of life.
