桃红四物汤加减对下肢骨折术后肿胀、炎症反应及血小板活化的影响观察

Observation on the Influence of Modified Taohong Siwu Decoction on Postoperative Swelling,Inflammatory Reaction and Platelet Activation of Lower Limb Fracture

目的:探究及观察桃红四物汤加减对下肢骨折术后肿胀、炎症反应及血小板活化的影响。方法:将2019年12月-2021年12月厦门市中医院的86例下肢骨折患者按照随机数字表法分为对照组43例和观察组43例。对照组进行常规下肢骨折术后治疗,观察组则在对照组的基础上加用桃红四物汤进行治疗。比较两组的肿胀治疗总有效率,治疗前后的肿胀程度、炎症反应指标[血清白介素-6(IL-6)、白介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)]及血小板活化指标[血小板α-颗粒膜蛋白(GMP-140)及血小板膜糖蛋白(CD61)]。结果:治疗1周及2周后观察组的肿胀治疗总有效率均显著高于对照组,差异均有统计学意义(P<0.05);治疗前两组的肿胀程度、炎症反应指标及血小板活化指标比较,差异均无统计学意义(P>0.05),治疗1周及2周后观察组的肿胀程度均显著优于对照组、血小板活化指标均显著低于对照组,治疗3、7及10 d后观察组的炎症反应指标均显著低于对照组,差异均有统计学意义(P<0.05)。结论:桃红四物汤加减可显著改善下肢骨折患者的术后肿胀、炎症反应及血小板活化状态,在下肢骨折患者中的应用价值较高。

Objective:To investigate and observe the influence of modified Taohong Siwu Decoction on postoperative swelling,inflammatory reaction and platelet activation of lower limb fracture.Method:A total of 86 patients with lower limb fracture in Xiamen Hospital of Traditional Chinese Medicine from December 2019 to December 2021 were divided into control group 43 cases and observation group 43 cases by the random number table.The control group was treated with routine postoperative treatment of lower limb fracture,the observation group was treated with modified Taohong Siwu Decoction on the basis of the control group.The total effective rates of swelling treatment were compared between the two groups.The swelling degree,inflammatory reaction indexes[serum interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)]and platelet activation indexes[plateletα-granule membrane protein 140(GMP-140)and platelet glycoprotein(CD61)]were compared between the two groups before and after treatment.Result:The total effective rates of swelling of observation group after 1 week and 2 weeks of treatment were significantly higher than those of control group,the differences were statistically significant(P<0.05);the swelling degree,inflammatory reaction indexes and platelet activation indexes of the two groups before the treatment were compared,the differences were not statistically significant(P>0.05),after 1 week and 2 weeks of treatment,the swelling degree of the observation group were significantly better than those of the control group,the platelet activation indexes were significantly lower than those of the control group,the inflammatory reaction indexes of the observation group were significantly lower than those of the control group after 3,7 and 10 days of treatment,the differences were statistically significant(P<0.05).Conclusion:The modified Taohong Siwu Decoction can significantly improve the postoperative swelling,inflammatory reaction and platelet activation state of patients with lower limb fracture,so its application value in the patients with lower limb fracture is higher.
