带状疱疹后遗神经痛患者实施改良胸椎旁神经阻滞麻醉治疗的临床效果

Clinical Effect of Modified Thoracic Paravertebral Nerve Block Anesthesia in Patients with Postherpetic Neuralgia

目的研究改良胸椎旁神经阻滞麻醉(modified thoracic paravertebral nerve block anesthesia,MPNBA)治疗带状疱疹后遗神经痛(postherpetic neuralgia,PHN)的临床效果。方法研究对象选取徐州医科大学附属宿迁医院于2019年6月-2021年3月收治的124例PHN患者,按随机数表法分为两组,各62例。对照组行硬膜外神经阻滞麻醉(epidural nerve block anesthesia,ENBA)治疗,观察组行MPNBA治疗,对比两组治疗效果、不良反应及治疗前后疼痛评分、睡眠质量评分。结果与对照组(82.26%)比较,观察组治疗总有效率(96.77%)更高,差异有统计学意义(χ^(2)=6.960,P<0.05);观察组治疗后VAS评分、PSQI评分分别为(1.34±0.21)分、(5.13±1.04)分,均低于对照组(2.25±0.62)分、(7.35±1.05)分,差异有统计学意义(P<0.05);与对照组(16.13%)比较,观察组不良反应发生率(3.22%)更低,差异有统计学意义(χ^(2)=5.904,P<0.05)。结论MPNBA用于PHN治疗,可促进疗效提高,提高镇痛效果,促进患者睡眠质量改善,且不良反应少,可确保治疗安全性。

Objective To study the clinical effect of modified thoracic paravertebral nerve block anesthesia(MPNBA)in the treatment of postherpetic neuralgia(PHN).Methods The research subjects were 124 PHN patients admitted to Suqian Hospital Affiliated to Xuzhou Medical University from June 2019 to March 2021.According to the random number table method,they were divided into two groups,each with 62 cases.The control group was treated with epidural nerve block anesthesia(ENBA),and the observation group was treated with modified thoracic paravertebral nerve block anesthesia.The therapeutic effects,adverse reactions,pain scores and sleep quality scores before and after treatment were compared between the two groups.Results Compared with the control group(82.26%),the observation group(96.77%)had a higher total effective rate,the differences were statistically significant(χ^(2)=6.960,P<0.05);the VAS score and PSQI score of the observation group after treatment were(1.34±0.21)points and(5.13±1.04)points,respectively,which were lower than those of the control group(2.25±0.62)points and(7.35±1.05)points,and the differences were statistically significant(P<0.05);compared with the control group(16.13%),the incidence of adverse reactions in the observation group(3.22%)was lower,the differences were statistically significant(χ^(2)=5.904,P<0.05).Conclusion MPNBA is used in the treatment of PHN,which can improve the curative effect,improve the analgesic effect,improve patient sleep quality,and has few adverse reactions,which can ensure the safety of the treatment.
