摘要
目的探讨在早产儿呼吸暂停(apnea of prematurity,AOP)中使用枸橼酸咖啡因治疗对内皮素(ET)及β-内啡肽(β-EP)水平的影响。方法选取2019年8月-2020年12月在深圳市福田区妇幼保健院接受治疗的60例早产儿AOP患者作为研究对象。根据随机数表法将患儿分为观察组(n=30)与对照组(n=30),对照组使用氨茶碱治疗,观察组使用枸橼酸咖啡因治疗,对两组ET、β-EP水平及动脉血气指标进行比较。结果治疗后,两组ET水平较治疗前明显提高,且观察组(62.76±8.25)ng/L高于对照组(53.64±7.37)ng/L,差异有统计学意义(t=4.515,P<0.05);治疗后,两组β-EP水平较治疗前明显下降,且观察组β-EP水平(93.54±11.35)ng/L明显低于对照组,差异有统计学意义(P<0.05);治疗后,两组动脉血二氧化碳分压(PaCO_(2))较治疗前明显下降,动脉血氧分压(PaO_(2))较治疗前明显提高,且观察组PaCO_(2)明显低于对照组,PaO_(2)明显高于对照组,差异有统计学意义(P<0.05)。结论将枸橼酸咖啡因应用于AOP患儿治疗中可改善机体ET与β-EP水平,效果较好。
Objective To investigate the effect of caffeine citrate treatment on endothelin(ET)and β-endorphin(β-EP)levels in premature infants with apnea of prematurity(AOP).Methods A total of 60 premature infants with AOP who were treated in Maternal and Child Health Hospital from August 2019 to December 2020 were selected as the research subjects.According to the random number table method,the children were divided into the observation group(n=30)and the control group(n=30).The control group was treated with aminophylline,and the observation group was treated with caffeine citrate.The levels of ET,β-EP and arterial blood gas were compared between the two groups.Results After treatment,the levels of ET in the two groups were significantly higher than those before treatment,and the observation group(62.76±8.25)ng/L was higher than the control group(53.64±7.37)ng/L,and the difference was statistically significant(t=4.515,P<0.05);after treatment,the levels ofβ-EP in the two groups were significantly lower than those before treatment,and the levels ofβ-EP in the observation group(93.54±11.35)ng/L were significantly lower than those in the control group,the difference was statistically significant(P<0.05);after treatment,the arterial partial pressure of carbon dioxide(PaCO_(2))in the two groups was significantly decreased compared with before treatment,the arterial partial pressure of oxygen(PaO_(2))was significantly increased compared with before treatment,and the PaCO_(2) of the observation group was significantly lower than that of the control group,the PaO_(2)of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion The application of caffeine citrate in the treatment of children with AOP can improve the levels of ET andβ-EP,and the effect is good.
