补中益气汤联合辨证刮痧对乳腺癌术后癌性疲劳患者生活质量及免疫功能的改善作用

Effect of Buzhong Yiqi Decoction Combined with Syndrome Differentiation-based Scraping on Quality of Life and Immune Function of Patients with Cancer-related Fatigue after Breast Cancer Surgery

目的探讨补中益气汤加辨证刮痧对乳腺癌(Breast cancer,BC)术后癌性疲劳(Cancer-realated fatigue,CRF)患者生活质量及免疫功能的改善作用。方法选取2018年8月—2020年10月期间衡水市中医医院收治的BC术后CRF患者106例,随机分为对照组和治疗组,每组各53例。对照组术后视患者具体情况予以综合抗癌方案,观察组术后加用补中益气汤+辨证刮痧治疗,两组患者均持续治疗4周。观察比较两组患者Piper疲乏量表(Piper fatigue sacale,PFS)、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)、Karnofsky评分标准(Karnofsky performance status,KPS)、生活质量综合评定问卷(generic quality of life inventory-74,GQOLI-74)评分及血清CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平。结果治疗后治疗组情感、行为及感觉维度评分均低于治疗前及对照组,差异均有统计学意义(P<0.05)。治疗后两组患者KPS评分高于治疗前,PSQI评分低于治疗前,差异有统计学意义(P<0.05);且治疗组KPS评分高于对照组,PSQI评分低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者GQOLI-74各维度评分均高于治疗前,差异有统计学意义(P<0.05);且治疗组GQOLI-74各维度评分高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,差异有统计学意义(P<0.05);且治疗组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,差异有统计学意义(P<0.05)。结论补中益气汤加辨证刮痧可有效改善BC术后CRF患者的疲劳症状、睡眠质量、健康状态、生活质量及免疫功能,值得临床推广。

Objective To explore the effect of Buzhong Yiqi Decoction combined with syndrome differentiation-based scraping on the quality of life and immune function of patients with cancer-related fatigue(CRF)after breast cancer(BC)surgery.Methods A total of 106 CRF patients after BC surgery who were admitted to the Hengshui Hospital of Traditional Chinese Medicine from August 2018 to October 2020 were enrolled and randomized into the control group(n=53)and the treatment group(n=53).Both groups received conventional comprehensive anti-cancer treatment after surgery,and the treatment group was added with Buzhong Yiqi Decoction and syndrome differentiation-based scraping.The treatment lasted 4 weeks for the two groups.The scores of Piper Fatigue Scale(PFS),Pittsburgh Sleep Quality Index(PSQI),Karnofsky Performance Status(KPS),and Generic Quality of Life Inventory-74(GQOLI-74)and the levels of serum CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)were compared between the two groups.Results The scores in emotions,behavior,and senses in PFS in the treatment group were lower than those before treatment and in the control group(P<0.05).After treatment,the KPS score and GQOLI-74 scores of the two groups were higher than those before treatment,and the PSQI score was lower than that before treatment(P<0.05).The scores of KPS and GQOLI-74 in the treatment group were higher than those in the control group,and the PSQI score was lower than that in the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the two groups were higher than those before treatment(P<0.05),and the levels of them in the treatment group were higher than those in the control group(P<0.05).Conclusion Buzhong Yiqi Decoction combined with syndrome differentiation-based scraping can improve the fatigue symptoms,sleepquality,health status,quality of life,and immune function of CRF patients after BC surgery,which is worthy of clinical promotion.
