血清PCT对就诊结直肠癌化疗后中性粒细胞缺乏并发细菌血流感染患者的诊断效果研究

Diagnostic effect of serum PCT in patients with neutropenia complicated with bacterial bloodstream infection after chemotherapy for colorectal cancer

目的 探讨血清降钙素原对就诊结直肠癌化疗后中性粒细胞缺乏并发细菌血流感染患者的诊断效果,以期为结直肠癌化疗后中性粒细胞缺乏并发细菌血流感染患者的诊断和治疗及改善患者预后提供理论依据。方法 选择自2017年6月-2020年3月在我院接受化疗治疗的结直肠癌化疗后中性粒细胞缺乏的患者442例,其中经过临床诊断发生细菌性血流感染的患者110例组成血流感染组,332例患者未发生血流感染为非血流感染组,对两组患者的临床资料进行统计、分析和比较。结果 结直肠癌化疗后中性粒细胞缺乏并发细菌血流感染患者的血清降钙素原表达水平高于非血流感染组;血流感染患者的中性粒细胞下降时间及中性粒细胞缺乏持续时间均长与非血流感染组,P<0.05;血清降钙素原检测的敏感度为97.05,特异性为88.77,均高于血清C-反应蛋白检测的72.23、39.45,P<0.05。结论 结直肠癌化疗后中性粒细胞缺乏患者的血流感染发病率较高;血清降钙素原用于诊断结直肠癌化疗后中性粒细胞缺乏并发细菌血流感染的敏感度和特异性较高,值得临床推广应用。

Objective To investigate the diagnostic effect of serum procalcitonin in patients with neutropenia complicated with bacterial bloodstream infection after chemotherapy of colon cancer, so as to provide theoretical basis for the diagnosis and treatment of neutropenia complicated with bacterial bloodstream infection after chemotherapy of colon cancer and to improve the prognosis of patients.Methods From June 2017 to March 2020,442 colorectal cancer patients with neutropenia after chemotherapy in our hospital were selected.Among them, 110 patients with bacterial bloodstream infection after clinical diagnosis were divided into bloodstream infection group and 332 patients without bloodstream infection were divided into non bloodstream infection group.The clinical data of the two groups were statistically analyzed and compared.Results The expression of procalcitonin in patients with neutropenia and bacterial bloodstream infection after chemotherapy was higher than that in patients without bloodstream infection;The duration of neutropenia and neutropenia in patients with bloodstream infection were longer than those in patients without bloodstream infection(P<0.05);The sensitivity and specificity of procalcitonin were 97.05 and 88.77 respectively, which were higher than 72.23 and 39.45 of serum C-reactive protein(P<0.05).Conclusion Hair coloring cancer has a higher incidence of blood stream infection after chemotherapy.Serum procalcitonin has high sensitivity and specificity in the diagnosis of neutropenia complicated with bacterial bloodstream infection after chemotherapy of colorectal cancer, which is worthy of clinical application.
