系统护理在高龄食管癌根治术病人ICU转出期间的应用

Systematic care during the transfer out of ICU for elderly patients undergoing radical esophageal cancer

目的:观察在食道癌根治术的老年患者转出ICU期间实施系统护理的效果。方法:选取2019年1月~2021年12月在济宁市两所综合三甲医院重症医学科接受根治性切除术的78名高龄食道癌患者,随机分为对照组(n=39)和研究组(n=39)。对照组给予常规护理,研究组给予系统护理。比较患者的引流管留置时间、吻合口愈合时间、术后住院时长、并发症发生率、视觉模拟量表(VAS)、重返重症监护室率、不良事件发生率、自评焦虑量表(SAS)、自评抑郁量表(SDS)和出院时的护理满意度。济宁市两所三甲医院重症医学科行食管癌根治术并入住重症监护室的高龄食管癌病人78例作为研究对象 结果:护理后,与对照组患者相比,研究组患者的引流管留置时间、吻合口愈合时间和术后住院时长更短,术后并发症发生率、重返ICU率和不良事件发生率更低,VAS评分、SAS评分和SDS评分也更低,差异具统计学意义。出院时对患者进行了调查,发现研究组患者的护理满意度明显好于对照组。结论:在食道癌根治术的老年患者转出ICU期间实施系统护理,具有良好的效果,有效促进患者康复,改善病人心理状态。

Objective Observe the effect of implementing systemic nursing care during the transfer out of ICU of elderly patients undergoing radical esophagectomy for esophageal cancer.Methods Seventy-eight elderly esophageal cancer patients who underwent radical resection in our hospital from January 2019 to December 2021 were selected and randomly divided into a control group (n=39) and a study group (n=39).The control group was given conventional care and the study group was given systematic care.The patients’ drainage tube retention time,anastomotic healing time,postoperative length of stay,complication rate,visual analog scale (VAS),return to intensive care unit rate,adverse event rate,self-rated anxiety scale (SAS),self-rated depression scale (SDS) and satisfaction with care at discharge were compared.Seventy-eight elderly esophageal cancer patients who underwent radical esophageal cancer surgery and were admitted to the intensive care unit in our department were used as the study subjects.Results After care,patients in the study group had shorter drainage tube retention time,anastomotic healing time and postoperative length of stay,lower postoperative complication rate,return to ICU rate and adverse event rate,and lower VAS score,SAS score and SDS score compared with patients in the control group,with statistically significant differences.Patients were surveyed at discharge and were found to have significantly better nursing satisfaction in the study group than in the control group.Conclusion Implementing systematic nursing care during the transfer out of ICU for elderly patients with radical esophageal cancer has good effects,effectively promoting patients’ recovery and improving their psychological status.
