BiPAP呼吸机泵雾化吸入治疗尘肺合并慢性Ⅱ型呼吸衰竭的疗效观察 被引量:2

摘要 目的 观察经双水平气道内正压(BiPAP)呼吸机泵驱动雾化吸入治疗尘肺合并慢性Ⅱ型呼吸衰竭患者的临床疗效。方法 将100例尘肺合并慢性Ⅱ型呼吸衰竭患者分为对照组50例与观察组50例。观察组使用改装装置连接BiPAP无创呼吸机与空气压缩雾化吸入装置,利用无创辅助通气(NPPV)吸气相正压泵入空气压缩雾化支气管扩张剂治疗;对照组使用传统方法,NPPV间歇期空气压缩雾化吸入支气管扩张剂治疗,比较2种治疗方法的临床疗效。结果 观察组治疗总有效率96%、对照组72%,2组间比较差异有统计学意义(P<0.05)。血气分析各项指标,观察组较对照组改善更加显著(P<0.05)。结论 BiPAP呼吸机泵雾化吸入治疗尘肺合并慢性Ⅱ型呼吸衰竭比传统NPPV间歇期空气压缩雾化吸入治疗的临床疗效更加显著,值得在临床广泛推广。 Objective To observe the clinical efficacy of atomized inhalation drived by BiPAP(Bi-Level Positive Airway Pressure)ventilator pump in the treatment of pneumoconiosis combined with the chronic type II respiratory failure.Methods 100 cases of pneumoconiosis combined with the chronic type II respiratory failure were divided into control group(50 cases)and observation group(50 cases).The modified device connected with BiPAP non-invasive ventilator and air compression atomization inhalation device, and air compression atomization bronchodilator was pumped into the positive pressure pump of NPPV(non-invasive auxiliary ventilation),was used in observation group.The control group was treat with the traditional methods of atomization inhalation of bronchodilator during NPPV interval.The clinical efficacies of these two treatments were compared.Results The total treatment effective rate was 96% in observation group and 72% in the control group.The difference between the two treatments was statistically significant(P<0.05).The indicators of blood gas analysis in the observation group were improved more significantly than those in the control group(P<0.05).Conclusion BiPAP non-invasive ventilator atomized inhalation is more effective than traditional methods in the treatment of pneumoconiosis complicated with chronic type Ⅱ respiratory failure, and it is worth widely popularized in clinical use.
