2型糖尿病合并胆石病与甲状腺功能相关性研究

摘要 目的探讨住院2型糖尿病患者合并胆石病的患病率及其与甲状腺功能的相关性。方法采用回顾性队列研究及倾向评分匹配方法。选取2017年1月—2021年12月于吉首大学第一附属医院内分泌科住院的2091例2型糖尿病患者,根据有无胆石病分为胆石病组(n=721)和非胆石病组(n=1370)。收集两组患者的性别、年龄、吸烟史、饮酒史、体质量、收缩压、舒张压等一般资料,检测FPG、HbA1c、空腹C-P、TG、TC、HDLC、LDL-C、TSH、FT3、FT4等生化指标。倾向评分匹配后比较两组患者甲状腺功能。甲状腺功能各指标与胆石病的Spearman相关性分析。结果倾向评分匹配后胆石病组与非胆石病组患者FT3[3.72(3.26,4.17)]pmol/Lvs[3.82(3.36,4.25)]pmol/L比较,差异有统计学意义(Z=2.781,P<0.05)。倾向评分匹配后,FT3与胆石病呈负相关(P<0.05)。结论2型糖尿病患者胆石病患病率与FT3呈负相关,但与FT4、TSH不相关。 Objective To investigate the prevalence of cholelithiasis in hospitalized patients with type 2 diabetes mellitus and its correlation with thyroid function.Methods Retrospective cohort studies and propensity score matching were used.A total of 2091 patients with type 2 diabetes who were hospitalized in the Endocrinology Department of the First Affiliated Hospital of Jishou University from January 2017 to December 2021 were selected.According to the presence or absence of cholelithiasis,the patients were divided into cholelithiasis group(n=721)and noncholelithiasis group(n=1370).The gender,age,smoking history,drinking history,weight,systolic blood pressure,diastolic blood pressure and other general data of the two groups of patients were collected,and biochemical indicators,such as FPG,HbA1c,fasting C-P,TG,TC,HDL-C,LDL-C,TSH,FT3,FT4,were detected Observation indicators:Comparison of thyroid function between two groups of patients after propensity score matching.Spearman correlation analysis between each index of thyroid function and cholelithiasis after propensity score matching.Results After propensity score matching,there was a statistically significant difference in FT3 between the cholelithiasis group and the non-cholelithiasis group[3.72(3.26,4.17)]pmol/L vs[3.82(3.36,4.25)]pmol/L(Z=2.781,P<0.05).After propensity score matching,FT3 was negatively correlated with cholelithiasis(P<0.05).Conclusion The prevalence of cholelithiasis in patients with type 2 diabetes is negatively correlated with FT3,but not with FT4 and TSH.
