大剂量半夏治疗脾虚胃热型慢性胃炎的临床疗效及其安全性评估

Clinical Efficacy and Safety Evaluation of High-dose Pinellia Ternata in the Treatment of Chronic Gastritis due to Spleen Deficiency and Stomach Heat

目的探讨大剂量半夏治疗脾虚胃热型慢性胃炎的临床疗效及其安全性评估。方法将东莞市东南部中心医院2020年1月—2021年1月收治的88例慢性胃炎患者,按随机数表法分为对照组和观察组,各44例,对照组给予奥美拉唑、铝碳酸镁西药治疗,观察组给予大剂量半夏的半夏泻心汤。比较两组患者的临床疗效,治疗前后胃痛、反酸、身体困重、胃脘痞闷、少食纳呆的中医症状评分,血清胃动素和胃泌素水平以及不良反应发生情况。结果观察组临床疗效总有效率(95.45%)高于对照组(79.55%),差异有统计学意义(χ^(2)=5.091,P<0.05);治疗后,观察组患者胃痛、反酸、身体困重、胃脘痞闷、少食纳呆评分低于对照组,血清胃动素和胃泌素水平高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论脾虚胃热型慢性胃炎患者应用大剂量半夏的半夏泻心汤,可有效改善患者临床症状和胃肠动力,降低不良反应发生率,安全性较高,疗效确切。

Objective To investigate the clinical efficacy and safety evaluation of high-dose Pinellia ternata in the treatment of chronic gastritis due to spleen deficiency and stomach heat.Methods A total of 88 patients with chronic gastritis admitted to Dongguan Southeast Central Hospital from January 2020 to January 2021 were divided into a control group and an observation group according to the random number table method,with 44 cases in each group.The control group was given omeprazole,aluminum and magnesium carbonate western medicine,and the observation group was given high-dose Banxia xiexin decoction.The clinical efficacy of the two groups of patients was compared,the TCM symptom scores of stomach pain,acid regurgitation,body sluggishness,epigastric fullness,poor appetite,serum motilin and gastrin levels,and adverse reactions before and after treatment were compared.Results The total effective rate of clinical efficacy in the observation group was 95.45%higher than that in the control group,79.55%,the difference was statistically significant(χ^(2)=5.091,P<0.05);after treatment,the score of the observation group in stomach pain,acid regurgitation,heavy body weight,stuffy stomach,and less appetite were lower than the control group,and the levels of motilin and gastrin in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion In patients with chronic gastritis of spleen deficiency and stomach heat,high-dose Banxia Xiexin Decoction can effectively improve the clinical symptoms and gastrointestinal motility.It can effectively improve the clinical symptoms and gastrointestinal motility of patients,reduce the incidence of adverse reactions,and has high safety and definite curative effect.
