肇庆市肾透明细胞癌的预后不良危险因素分析

Analysis of risk factors for poor prognosis of renal clear cell carcinoma in Zhaoqing city

目的探讨肇庆市肾透明细胞癌的预后不良危险因素.方法以2014年-2018年在肇庆市怀集县人民医院治疗的肾透明细胞癌患者176例为观察对象.采用资料搜集、临床检查、病理结果和随访预后四个步骤展开研究,根据患者肿瘤特异性生存(CSS)终点事件对患者进行分组,对不同组患者收集到的数据进行对比分析.结果176名患者中共有33人死于肿瘤特定疾病,被纳入死亡组,其余143例被纳入存活组.死亡组与存活组患者在年龄、吸烟史、酗酒史、婚姻状况、同型半胱氨酸、病理分级、分期方面差异有统计学意义(P<0.05).其余指标方面两组患者差异无统计学意义(P>0.05).Logistic回归方程分析后,结果显示年龄、同型半胱氨酸、病理分级、临床分期是患者死于透明细胞癌特定疾病的独立危险因素.结论年龄、同型半胱氨酸、病理分级、临床分期是患者死于透明细胞癌特定疾病的独立危险因素,对于具有上述高风险因素的患者,应重视其预后.

Objective To explore the risk factors of poor prognosis of renal clear cell carcinoma in zhaoqing.Methods 176 patients with renal clear cell carcinoma treated in our hospital from 2014 to 2018 were observed.The patients were divided into four groups according to the pathological examination and prognosis,and the patient survival data were collected according to the four steps of CSS.Results Of the 176 patients,33 died of tumor specific diseases and were included in the death group,while the remaining 143 were included in the survival group.There were significant differences in age,smoking history,alcoholism history,marital status,homocysteine,pathological grade and stage between the death group and the survival group(P<0.05).There was no significant difference in other indexes between the two groups(P>0.05).Logistic regression equation showed that age,homocysteine,pathological grade and clinical stage were independent risk factors for patients to die of specific diseases of clear cell carcinoma.Conclus ions Age,homocysteine,pathological grade and clinical stage are independent risk factors for patients to die of specific diseases of clear cell carcinoma.For patients with the above high-risk factors,we should pay attention to their prognosis.
