3D-Slicer虚拟现实技术在神经外科脑肿瘤术前计划中的应用分析

摘要 目的探讨3D-Slicer虚拟现实技术在神经外科脑肿瘤术前计划中的应用价值。方法18例行手术治疗的脑肿瘤患者,采用3D-Slicer虚拟现实技术实施三维重建,实施手术模拟与设计手术入路。结果18例患者均按照最佳模拟手术方案,成功完成术前计划与模拟手术操作,且顺利按照计划完成手术。所有患者实际的手术情况和模拟手术中所见处于完全吻合状态,通过借助显微镜与内镜下将肿瘤完全切除;术后各项临床症状均得到好转,无新发症状出现,且术后无患者出现死亡,无视力下降、脑膜炎等并发症发生;经术后3个月复查,头颅磁共振成像(MRI)增强结果显示无肿瘤复发。结论通过在术前进行3D-Slicer软件虚拟重建,制定出详细手术计划,能够得到大量的立体解剖相关信息,以此作为指导,可帮助操作者设计出手术切口、手术入路,顺利切除肿瘤,不仅能够有效提升患者手术治疗成功率,减少术后并发症的发生,还能够起到保护患者脑功能的作用,适宜临床深入研究与推广应用。 Objective To discuss the practical value of 3D-Slicer virtual reality technology in preoperative planning of neurosurgical brain tumors.Methods 18 patients with brain tumors treated surgically were treated with 3D-Slicer virtual reality technology to implement 3 d reconstruction,perform surgical simulation and design surgical approaches.Results All 18 patients successfully completed the preoperative planning and simulated surgical operation according to the best simulated surgical regimen,and the surgery was successfully completed according to the regimen.The actual surgical conditions of all patients were completely consistent with what was seen in the simulated surgery,and the tumor was completely removed by microscopy and endoscopy;all clinical symptoms improved after surgery,and no new symptoms appeared,and no patient died after surgery,and no complications such as vision loss and meningitis occurred;the enhanced magnetic resonance imaging(MRI)results showed no tumor recurrence after the postoperative review at 3 months.Conclusion By performing virtual reconstruction with 3D-Slicer software before surgery and formulating a detailed surgical regimen,a large amount of information related to stereotaxic anatomy can be obtained,which can be used as a guide to help the operator design the surgical incision and surgical access and remove the tumor smoothly.It can not only effectively improve the success rate of surgical treatment and reduce the occurrence of postoperative complications,but also protect the brain function of patients,which is suitable for in-depth clinical research and application.
