经尿道柱状水囊前列腺扩开术治疗BPH的疗效及安全性研究

Study on efficacy and safety of transurethral columnar balloon dilation of the prostate in the treatment of BPH

目的探究经尿道柱状水囊前列腺扩开术(TUCBDP)治疗良性前列腺增生(BPH)的疗效及安全性。方法250例良性前列腺增生患者作为研究对象,均行经尿道柱状水囊前列腺扩开术治疗。比较250例患者术前、术后12个月的最大尿流率(Qmax)、国际前列腺症状评分(IPSS)、膀胱残余尿量(PVR)、生活质量评分(QOL);比较50例保留性功能患者术前、术后12个月的勃起硬度评估(EHS)等级、国际勃起功能评分(IIEF-5)。结果术后12个月,250例患者的Qmax(30.9±3.1)ml/s大于术前的(5.9±1.7)ml/s,IPSS评分(7.15±1.20)分、PVR(14.9±7.1)ml、QOL评分(2.61±0.8)分均小于术前的(27.11±2.18)分、(66.9±10.9)ml、(4.98±0.6)分,差异具有统计学意义(P<0.05)。50例保留性功能患者术前、术后12个月的EHS等级、IIEF-5评分比较,差异无统计学意义(P>0.05)。结论经尿道柱状水囊前列腺扩开术治疗良性前列腺增生的效果显著,具有出血量少及手术时间短等优势,对保留性功能及高龄的患者效果显著,值得在临床中被广泛推广和应用。

Objective To investigate the efficacy and safety of transurethral columnar balloon dilation of the prostate(TUCBDP)in the treatment of benign prostatic hyperplasia(BPH).Methods 250 patients with benign prostatic hyperplasia were studied and all were treated with transurethral columnar balloon dilation of the prostate.The maximum urinary flow rate(Qmax),international prostate symptom score(IPSS),post voiding residual volume(PVR),and quality of life score(QOL)in 250 patients before and 12 months after surgery were compared.The erectile hardness evaluation(EHS)grading and international index of eretile function-5(IIEF-5)were compared before and 12 months after surgery in 50 patients with preserved sexual function.Results At 12 months after surgery,the Qmax(30.9±3.1)ml/s of 250 patients was greater than the preoperative(5.9±1.7)ml/s;the IPSS score(7.15±1.20)points,PVR(14.9±7.1)ml,and QOL score(2.61±0.8)points were all lower than the preoperative(27.11±2.18)points,(66.9±10.9)ml,and(4.98±0.6)points before surgery;the differences were all statistically significant(P<0.05).There was no statistically significant difference in the EHS grading and IIEF-5 score of 50 patients with preserved function before surgery and 12 months after surgery(P>0.05).Conclusion Transurethral columnar balloon dilation of the prostate for BPH is effective,with the advantages of less blood loss and shorter operation time,and is effective in preserving sexual function and in patients of advanced age,which is worthy of clinical promotion and application.
