逐瘀通痹汤治疗膝关节骨性关节炎的临床研究

Clinical study of Zhuyu Tongbi decoction in treating knee osteoarthritis

目的 观察逐瘀通痹汤治疗膝关节骨性关节炎效果。方法 纳入新里程安钢总医院2018年4月—2021年2月诊治的膝关节骨性关节炎患者88例,以随机数字表法分为两组,各44例。对照组西药治疗,观察组联合逐瘀通痹汤,均治疗12周。对比两组疗效、膝关节结构与功能及炎性因子水平。结果 观察组治疗总有效率93.18%较对照组77.27%高,两组治疗后WOMAC各项及总评分、炎性因子水平均下降,且观察组僵硬、疼痛、日常活动困难度、WOMAC总分[(2.08±0.41)分、(3.47±0.82)分、(18.23±3.35)分、(23.30±4.52)分]及IL-1β、TNF-α[(11.10±3.27)pg·mL、(26.24±4.90)pg·mL]水平均较对照组[(3.71±0.65)分、(7.36±1.90)分、(24.40±5.66)分、(35.41±5.08)分、(14.85±3.63)pg·mL、(32.93±5.16)pg·mL、]低(P<0.05)。结论 膝关节骨性关节炎患者应用逐瘀通痹汤治疗可改善临床症状,降低炎症因子水平,促进膝关节功能恢复。

Objective To observe the effects of Zhuyu Tongbi decoction in treating knee osteoarthritis.Methods Totally 88 patients with knee osteoarthritis diagnosed and treated in our hospital from April 2018 to February 2021 were included.They were divided into two groups by random number table method, with 44 cases each.Control group was treated with western medicine, while observation group combined with Zhuyu Tongbi decoction, both were treated for 12 weeks.The efficacy, knee joint structure and function and levels of inflammatory factors in articular fluid in both groups were compared before and after treatment.Results Compared the total effective rate 77.27% in the control group, the observation group 93.18% was higher.Each item and total scores of WOMAC and level of inflammatory factors in articular fluid were decreased in both groups, and levels of stiffness, pain, daily activity difficulty, WOMAC total score, IL-1β and TNF-α in articular fluid in observation group [(2.08±0.41),(3.47±0.82),(18.23±3.35),(23.30±4.52),(11.10±3.27) pg/mL,(26.24±4.90) pg/mL] were lower than control group [(3.71±0.65),(7.36±1.90),(24.40±5.66),(35.41±5.08),(14.85±3.63) pg/mL,(32.93±5.16) pg/mL](P<0.05).Conclusion The application of Zhuyu Tongbi Decoction in the treatment of knee osteoarthritis can improve the clinical symptoms, reduce the level of inflammatory factors and promote the recovery of knee function.
