大疱性类天疱疮患者外周血嗜酸粒细胞计数与抗体的相关性

The relationship between blood eosinophils and autoantibodies in patients with bullous pemphigoid

目的分析大疱性类天疱疮患者外周血嗜酸粒细胞计数与血清抗BP180IgG抗体、抗BP230IgG抗体、抗表皮基膜IgG抗体滴度(即间接免疫荧光滴度)及与血清总IgE值的关系。方法收集沈阳市第七人民医院2014年1月-2020年7月大疱性类天疱疮患者病例资料进行回顾性分析,根据抗BP180IgG抗体、抗BP230IgG抗体阳性情况将患者分组,比较组间外周血嗜酸粒细胞计数的差异。同时分析外周血嗜酸粒细胞计数与抗表皮基膜IgG抗体滴度及血清总IgE值的相关性。结果共收集154例患者。抗BP230IgG抗体阳性伴抗BP180IgG抗体阳性组外周血嗜酸粒细胞计数显著增高,且与抗BP180IgG抗体及抗BP230IgG抗体均阴性组及仅抗BP180IgG抗体阳性组进行比较,外周血嗜酸粒细胞计数差异均有统计学意义(U=189.5,Z=-4.410,P<0.001;U=1544.5,Z=-2.813,P=0.005);而抗BP180IgG抗体及抗BP230IgG抗体均阴性组与仅抗BP180IgG抗体阳性组相比较,外周血嗜酸粒细胞计数差异无统计学意义(U=331.000,Z=-2.199,P=0.028)。外周血嗜酸粒细胞计数与抗表皮基膜IgG抗体滴度及血清总IgE值均呈正线性相关(r=0.310,P<0.001 r=0.399,P<0.001)。结论外周血嗜酸粒细胞计数与抗表皮基膜IgG抗体水平呈正线性相关,且抗BP230IgG抗体阳性时外周血嗜酸粒细胞计数增高更显著,但未发现抗BP180IgG抗体阳性情况对外周血嗜酸粒细胞计数的影响。外周血嗜酸粒细胞计数与血清总IgE值正线性相关。

Objective To analyze the relationships between circulating eosinophil numbers and anti-BP180IgG antibody,anti-BP230IgG antibody,anti-basal membrane zone IgG antibody titers,total serum IgE levels in patients with bullous pemphigoid.Methods Cases of bullous pemphigoid in Shenyang Seventh People's Hospital from January 2014 to July 2020 were collected for retrospective study.The patients were divided into three groups according to the positive conditions of anti-BP180IgG antibody and anti-BP230lgG antibody.The differences in circulating eosinophil numbers among groups were compared.At the same time,the correlations between circulating eosinophil numbers and anti-basal membrane zone IgG antibody titers,total serum IgE levels were analyzed.ResultsA total of 154 patients were collected.Circulating eosinophil numbers are significantly higher in the group with anti-BP230IgG antibody and anti-BP180IgG antibody are both positive,compared with circulating eosinophil numbers in the group with anti-BP180IgG antibody,anti-BP230IgG antibody are both negative or the group with anti-BP180IgG antibody positive only,there is significant difference respectively(U=189.5,Z=-4.410,P<0.001,U=1544.5,Z=-2.813,P=0.005);But there is no significant difference between circulating eosinophil numbers in the group with anti-BP180IgG antibody,anti-BP230IgG antibody are both negative and the group with anti-BP180IgG antibody positive only(U-331.000,Z--2.199,P-0.028).There is a positive linear correlation between circulating eosinophil numbers and anti-basal membrane zone IgG antibody titers or total serum IgE levels respectively(r-0.310,P<0.001,r-0.399,P<0.001).Conclusions There is a positive linear correlation between circulating eosinophil numbers and Anti-basal membrane zone IgG antibody titers.Circulating eosinophil numbers are significantly higher in patients with anti-BP230IgG antibody positive.However,we didn't find the relationship between circulating eosinophil numbers and anti-BP180lgG antibody positive or not.There is a positive linear correlation between circulating eosinophil numbers and serum total IgE levels..
