5A护理模式用于缺血性脑卒中患者效果评价

Application Effect of 5A Nursing Mode in Patients with Ischemic Stroke

目的探讨5A护理模式应用于缺血性脑卒中的效果。方法选取医院2019年1月至12月收治的缺血性脑卒中患者400例,以2019年1月至6月收治的患者200例为对照组,行常规管理;以2019年7月至12月收治的患者200例为观察组,行5A护理模式管理。采用中文版Morisky服药依从性量表(MMAS)评估服药依从性,采用一般自我效能感量表评估一般自我效能感,采用Barthel(BI)指数评定量表评估日常生活活动能力,分别计算患者的血压和血糖达标率,并分析服药依从性、自我效能感、血压达标率、血糖达标率与日常生活活动能力的相关性。结果观察组患者治疗3,6,9,12个月的MMSA-8评分、一般自我效能感量表评分及BI指数评定量表评分均显著高于对照组(P<0.01);观察组患者的血压和血糖达标率分别为98%和96%,均显著高于对照组的84%和78%(P<0.05);服药依从性、血压达标率、血糖达标率、自我效能感与患者日常生活活动能力均呈正相关(r=0.207,0.188,0.117,0.215,P<0.001)。结论5A护理模式能提高缺血性脑卒中患者的自我健康管理能力,维持较好的服药依从性,增强其一般自我效能感。

Objective To investigate the application effect of 5A nursing mode in patients with ischemic stroke.Methods A total of 400 patients with ischemic stroke admitted to the hospital from January to December 2019 were selected and divided into the control group(from January to June 2019,routine management)and the observation group(from July to December 2019,5A nursing mode management).The Chinese version of Morisky Medication Compliance Scale(MMAS)was used to evaluate medication compliance,the General Self-Efficacy Scale was used to evaluate the general self-efficacy,and the Barthel(BI)index was used to evaluate the activities of daily living.The blood pressure and blood glucose compliance rate of patients were calculated respectively,and the correlation between medication compliance,self-efficacy,blood pressure compliance rate,blood glucose compliance rate and activities of daily living was analyzed.Results The scores of MMSA-8,General Self-Efficacy Scale and BI index in the observation group were significantly higher than those in the control group at 3,6,9 and 12 months after treatment(P<0.01).The blood pressure and blood glucose compliance rates in the observation group were 98%and 96%,which were significantly higher than 84%and 78%in the control group(P<0.05).Medication compliance,blood pressure compliance rate,blood glucose compliance rate and self-efficacy were all positively correlated with patients′activities of daily living(r=0.207,0.188,0.117,0.215,P<0.001).Conclusion 5A nursing model can improve the self-health management ability of patients with ischemic stroke,maintain better medication compliance and enhance their general self-efficacy.
