健康教练技术与互联网整合慢性病管理模式对高血压患者自我管理行为、疾病知识与血压控制的影响

Effects of Health Coaching Combined with Internet-Integrated Chronic Disease Management Mode on Self-Management Behavior,Disease Knowledge and Blood Pressure Control of Hypertensive Patients

目的探讨健康教练技术与互联网整合慢性病管理模式对高血压患者自我管理行为、疾病知识与血压控制的影响。方法选取医院2020年7月至2021年7月收治的高血压患者120例,按护理模式的不同分为对照组和研究组。对照组采用常规护理,研究组采用健康教练技术与互联网整合慢性病管理模式进行联合干预。结果与护理前比较,两组患者护理后的自我管理行为评分均显著升高,且研究组显著高于对照组(P<0.05);两组患者护理后的疾病知识掌握水平均显著升高,且研究组显著高于对照组(P<0.05);两组患者护理后的血压指标均显著改善,且研究组显著优于对照组(P<0.05);两组患者护理后的生活质量均显著升高,且研究组显著优于对照组(P<0.05);研究组护理满意度显著高于对照组(P<0.05)。结论在高血压患者护理期间,通过应用健康教练技术与互联网整合慢性病管理模式进行联合干预,有利于促进患者的康复。

Objective To investigate the effect of the health coaching combined with Internet-integrated chronic disease management mode on self-management behavior,disease knowledge and blood pressure control with hypertension patients.Methods A total of 120 patients with hypertension admitted to the hospital from July 2020 to July 2021 were selected and divided into the control group and the study group according to the different nursing modes.Patients in the control group were given the routine nursing,while patients in the study group were given the health coaching combined with Internet-integrated chronic disease management mode.Results Compared with that before nursing,the self-management behavior score after nursing in the two groups increased significantly,and that in the study group was significantly higher than that in the control group(P<0.05).Compared with that before nursing,the level of mastering disease knowledge after nursing in the two groups increased significantly,and that in the study group was significantly higher than that in the control group(P<0.05).Compared with that before nursing,the blood pressure after nursing in the two groups improved significantly,and that in the study group was significantly better than that in the control group(P<0.05).Compared with that before nursing,the quality of life after nursing in the two groups improved significantly,and that in the study group was significantly better than that in the control group(P<0.05).The nursing satisfaction in the study group was significantly higher than that in the control group(P<0.05).Conclusion During the nuring of patients with hypertension,the application of the health coaching combined with Internet-integrated chronic disease management mode can promote the recovery of them.
