血清CRP、PCT及病原菌联合检测对新生儿肺炎的临床价值分析

摘要 目的探讨血清C反应蛋白(CRP)、降钙素原(PCT)及病原菌联合检测对新生儿肺炎的临床价值。方法收集该院2020年2月至2021年3月收治的84例新生儿为研究对象,其中确诊为新生儿肺炎54例为实验组,其余30例为对照组;通过电化学发光法检测CRP及PCT浓度;病原菌检测通过采取患儿的痰液,进行痰培养后细菌分离,并通过细菌检测仪进行分析。结果实验组54例新生儿肺炎中27例(50.00%)检测出存在细菌感染,其中革兰阴性菌有17例,革兰阳性菌有10例。新生儿肺炎血清CRP及PCT值分别为(22.47±1.43)μg/mL、(1.62±0.17)ng/mL,均显著高于对照组(5.16±0.48)μg/mL、(0.08±0.01)ng/mL,差异有统计学意义(P<0.05)。新生儿肺炎治疗后血清CRP值为(7.31±0.47)μg/mL,明显低于治疗前,差异有统计学意义(P<0.05);新生儿肺炎治疗后PCT值为(0.36±0.04)ng/mL,明显低于治疗前,差异有统计学意义(P<0.05)。新生儿肺炎血清CRP、PCT及病原菌联合检测诊断阳性率为92.59%,显著高于单一使用CRP(87.04%)、PCT(62.96%)及病原菌检测(50.00%)的诊断效能,其χ^(2)分别为28.469、7.208、4.24,差异有统计学意义(P<0.05)。结论病原菌检测有助于患儿的病原学诊断,但指导诊疗的效能较晚;CRP及PCT有利于早期指导新生儿肺炎的抗生素规范使用,可与病原菌检测相辅相成,但其易受治疗的影响。新生儿肺炎血清CRP、PCT及病原菌联合检测可早期提高患儿的诊疗效能,值得临床推广应用。 Objective To study the clinical value of combined detection of serum CRP,PCT and pathogenic bacteria in children with neonatal pneumonia.Methods A total of 84 newborns admitted to our hospital from February 2020 to March 2021 were selected as the research objects.Among them,54 cases of neonatal pneu-monia were diagnosed as the experimental team,and the remaining 30 remainder were used as the control team.The concentration of CRP and PCT was inspected by electrochemiluminescence method.Pathogenic bac-teria detection is performed by taking the sputum of the child,separating the bacteria after sputum culture,and analyzing it with a bacteria detector.Results Of the 54 neonatal pneumonia in the experimental team,27 cases(50.00%)were detected with bacterial infection,of which 17 were gram-negative bacteria and 10 were gram-positive bacteria.Neonatal pneumonia serum CRP was 22.47±1.43μg/mL,PCT was 1.62±0.17ng/mL,which were higher than those of the control team(5.16±0.48μg/mL,0.08±0.01ng/mL),P<0.05,Se-rum CPR after treatment of neonatal pneumonia was 7.31±0.47μg/mL,which was lower than before treat-ment,P<0.05;PCT was 0.36±0.04ng/mL after treatment of neonatal pneumonia was significantly lower than before treatment,P<0.05.The positive rate of combined detection of CRP,procalcitonin and pathogenic bacteria in neonatal pneumonia was 92.59%,which was significantly higher than that of CRP(87.04%),PCT(62.96%)and pathogenic bacteria(50.00%).The efficiency,its chi-square values were 28.469,7.208,4.24,and P were all less than 0.05,the difference was statistically significant.Conclusion The combined detection of serum CRP,PCT,and pathogenic bacteria in neonatal pneumonia can improve the diagnostic efficacy of chil-dren in an early stage,which is worthy of clinical application.
