湖北武汉地区5559例HPV感染亚型分型及男女分型差异分析 被引量:1

摘要 目的调查湖北武汉地区人群感染人乳头瘤病毒(HPV)基因型分布情况,并对不同性别和年龄HPV感染情况进行差异分析。方法回顾性分析2021年1-8月在武汉大学中南医院皮肤科门诊送检的443例男性外生殖器刮擦物样本,与同期送检的5116例女性宫颈分泌物样本的14种高危HPV和5种低危HPV的检测结果,进行差异比较和统计分析。结果443例男性外生殖器刮擦物样本中HPV检出率为37.5%,其中低危型HPV-6和11的检出率最高(29.8%),高危型HPV检出前4位为HPV16、51、58和39。5116例女性宫颈分泌物样本中HPV检出率为20.2%,明显低于男性,差异有统计学意义(P<0.05),高危型HPV检出前4位为HPV-52、16、58和83。感染型别中以单一型感染最高,男性为75.0%,女性为69.0%,二重到五重及以上类型感染逐渐减少。女性各年龄组间HPV检出率差异有统计学意义(P<0.05),不同年龄组女性仅在HPV-18、33和11型检出率差异有统计学意义(P<0.05),而男性各年龄组间HPV感染率和HPV基因分布之间比较差异无统计学意义(P>0.05)。结论武汉地区两性HPV感染均以单一型感染为主,男性HPV感染情况和基因型分布与女性之间存在差异,不同年龄段女性感染HPV基因型不同,为本地区高危HPV的筛查和预防提供依据。 Objective To investigate the distribution of HPV genotypes,and to analyze the differences of HPV infection of different gender and age among patients in Wuhan,Hubei Province.Methods The HPV results of 443 male external genital scraps and 5116 female cervical secretions which were detected in Zhongnan Hospital of Wuhan University from January to August 2021,were analyzed retrospectively.The differences of the results were compared and statistically analyzed.Results The positive rate of HPV in 443 male external genital scraps was 37.5%,among which the positive rates of low-risk HPV6 and 11 were the highest(29.8%),and the top four high-risk HPV were HPV16,51,58 and 39.The positive rate of HPV in 5116 female cervical secretion samples was 20.2%,which was significantly lower than that in men,the difference was statistically significant(P<0.05).The top four high-risk HPV were HPV52,16,58 and 83.Among all of the infection types,single type infection was the highest,75%in men and 69%in women.The number of double to five and above types of infection decreased gradually.There were differences in the positive rate of HPV among different female age groups,the differences were statistically significant(P<0.05),and there were differences only in the positive rate of HPV18,33 and 11 in women of different age groups,the differences were statistically significant(P<0.05),but there was no significant difference in HPV infection rate and HPV genotype distribution among different male age groups(P>0.05).Conclusion The genotype of HPV infection in both sexes is dominated by single infection type in Wuhan.There are differences in HPV infection and genotype distribution between men and women.HPV genotypes are different in women of different ages.All of these above results provides a basis for screening and prevention of high-risk HPV in this area.
