鼻部用药枕在鼻内镜下鼻腔泪囊吻合手术后用药效果分析

Efficacy of nasal medicine pillow after lacrimal sac anastomosis under nasal endoscope

目的探讨鼻部用药枕在鼻内镜下鼻腔泪囊吻合术后的用药效果。方法采用便利抽样法将收治的96例(96只眼)鼻内镜下行鼻腔泪囊吻合手术患者,随机分为对照组和实验组,每组48只眼。对照组采用常规护理;实验组在常规护理的基础上,采用鼻部用药枕辅助用药。分别在术后1周比较两组患者在术后出血持续时间,术后4周、8周、12周比较两组患者在术后吻合口愈合状态,鼻部用药舒适度方面的差异。结果用鼻部用药枕实验组术后平均口腔吐出血时间(2.66±0.69)d、鼻腔分泌物出血时间(2.77±0.85)d均短于对照组,4周(1.60±0.64)、8周(1.35±0.56)、12周(1.10±0.31)吻合伤口愈合状态平均得分均优于对照组,颈椎(2.46±1.15)、鼻部(2.56±1.09)、面部(2.52±0.99)和头部(2.69±1.17)的用药舒适度有明显改善,差异均统计学意义(P<05)。结论本研究采用新型鼻部用药枕辅助给药,克服了术后患者用药由于体位和时间带来的问题,帮助患者摆放舒适和正确的鼻部用药体位,保证用药量,减少药物流失,提升了鼻腔泪囊吻合术后患者用药效果,值得临床推广应用。

Objective To investigate the effect of nasal medicine pillow after lacrimal sac anastomosis under nasal endoscope.Methods A prospective case-control study.A total of 96 patients(96 eyes)who received dacryocyst anastomosis under nasal endoscopy were collected by convenience sampling method,and randomly divided into control group(48 eyes)and experimental group(48 eyes).Conventional care was used in the control group.On the basis of routine nursing,the experimental group was used nasal pillow auxiliary drugs.The differences in the duration of postoperative bleeding at the first postoperative week,and in the healing status of the anastomosis and the comfort of nasal medication at the fourth,eighth and twelfth postoperative weeks between the two groups were compared.Results The mean postoperative oral bleeding time(2.66±0.69)and nasal discharge bleeding time(2.77±0.85)were shorter in the experimental group with nasal medicated pillows as compared with the control group,and the mean anastomotic wound healing status scores at four weeks(1.60±0.64),eight weeks(1.35±0.56),and twelve weeks(1.10±0.31)were better than in the control group,and the cervical spine(2.46±1.15),nasal(2.56±1.09),facial(2.52±0.99),and head(2.69±1.17)medication comfort was significantly improved as compared to the control group(P<0.05).Conclusions In this study,a new type of nasal pillow-assisted drug delivery was adopted,which overcame the problems caused by the position and time of drug delivery for postoperative patients,and helped patients to set up a comfortable and correct nasal drug delivery position,ensured the dosage,reduces the drug loss and improved the drug delivery effect of the patients after nasolacrimal sac anastomosis,which was worthy of clinical promotion and application.
