加味附子理中汤联合穴位敷贴治疗溃疡性结肠炎临床观察

Clinical observation on treating ulcerative colitis with the Fuzi Lizhong decoction plus acupoint sticking

目的:观察加味附子理中汤联合穴位敷贴治疗溃疡性结肠炎的临床疗效。方法:将选取的72例脾肾阳虚型溃疡性结肠炎患者按照随机数字表法分为对照组和治疗组,每组各36例。对照组予以常规西药治疗,治疗组在对照组基础上加用加味附子理中汤联合穴位敷贴治疗,两组疗程均为8周。观察并记录治疗前后两组中医证候积分、疾病活动度指数(改良Mayo评分、Baron内镜下溃疡性结肠炎活动分级评分)、血清炎症因子(肿瘤坏死因子-α、白细胞介素-6、C反应蛋白)、免疫功能指标(血清免疫球蛋白M、免疫球蛋白G、淋巴细胞/单核细胞值)。结果:经过8周的治疗,治疗组总有效率显著高于对照组(P<0.05);治疗后,两组患者中医证候积分、改良Mayo评分及Baron内镜下溃疡性结肠炎活动分级评分、血清炎症因子水平及淋巴细胞/单核细胞值均显著下降,免疫球蛋白M、免疫球蛋白G水平显著升高,差异均有统计学意义(P<0.05),且治疗组均优于对照组(P<0.05)。结论:加味附子理中汤联合穴位敷贴治疗溃疡性结肠炎临床疗效显著,可有效改善各项症状,促进患者肠黏膜的修复,同时具有减轻血清外周炎症因子水平及调节患者免疫功能的作用,二者联合应用值得临床推广。

Objective:To observe the clinical effect of the Fuzi Lizhong decoction(附子理中汤)plus acupoint sticking on ulcerative colitis.Methods:A total of 72 cases were divided into two groups according to the random number table method,with 36 cases in each group.The control group was treated with conventional Western medicine,and the treatment group was additionally treated with the Fuzi Lizhong decoction plus acupoint sticking.The course of treatment in both groups was 8 weeks.Results:After 8 weeks of treatment,the total efficiency in the treatment group was significantly higher than the control group(P<0.05).After treatment,the TCM syndrome scores,modified Mayo score,Baron endoscopic ulcerative colitis activity grading score,serum inflammatory factor levels,and lymphocyte/monocyte value were significantly decreased in the two groups.The levels of immunoglobulin M and immunoglobulin G were significantly increased,and the differences were all statistically significant(P<0.05).The treatment group was better than the control group(P<0.05).Conclusion:The Fuzi Lizhong decoction plus acupoint sticking shows a significant clinical effect on ulcerative colitis,can effectively ameliorate various symptoms,promoting the repair of the intestinal mucosa,reducing the level of peripheral blood inflammatory factors and regulate the immune function of the patient.The combined application of the two is worthy of clinical promotion.
