摘要
目的探讨卵泡液中生殖激素水平和卵泡大小与卵母细胞发育潜能的关系。方法一对一搜集48例行胞浆内单精子注射(ICSI)助孕患者的636份卵泡液,测量各卵泡直径[直径≥18 mm(大卵泡),14~<18 mm(中卵泡),<14 mm(小卵泡)]并记录,测定卵泡液中促卵泡生成素(FSH)、促黄体生成素(LH)、雌二醇(E2)、睾酮(T)水平,拆蛋后记录获取的卵母细胞的成熟度[生发泡期(GV)、第一次减数分裂中期(MⅠ)、第二次减数分裂中期(MⅡ)]。对444枚MⅡ卵行ICSI助孕,观察实验室结局。结果MⅡ组FSH水平显著高于GV组和MⅠ组,差异有统计学意义(P<0.05);GV组FSH水平与MⅠ组比较,差异有统计学意义(P<0.05)。大卵泡组、中卵泡组和小卵泡组FSH、LH、E2、T水平比较,差异无统计学意义(P>0.05)。大卵泡组双原核受精率高于小卵泡组和中卵泡组,差异有统计学意义(P<0.05);大卵泡组可利用胚胎率高于小卵泡组和中卵泡组,差异有统计学意义(P<0.05)。结论卵泡液中FSH、LH、T水平与卵母细胞成熟度密切相关,直径≥18 mm的卵泡获取的MⅡ卵发育潜能优于直径<18 mm的卵泡。
Objective To investigate the correlation of follicular fluid hormone levels and follicle size with oocyte developmental potential.Methods Follicular fluid of 636 follicles were collected from 48 patients undergoing ICSI treatment and all the follicle size were measured and recorded[diameter≥18 mm(big follicle group),14-<18 mm(middle follicle group),<14 mm(small follicle group)].Maturity of the oocytes was recorded[(germinal vesicle,GV),(metaphaseⅠ,MⅠ),(metaphaseⅡ,MⅡ)]and the concentrations of follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),testosterone(T)were measured.444 MⅡeggs were treatmented with ICSI and laboratory outcomes.Results The level of FSH in MⅡgroup was significantly higher than that in GV group and MⅠgroup,the difference was statistically significant(P<0.05).The level of FSH in GV group was significantly higher than that in MⅠgroup(P<0.05).There was no significant difference in the levels of FSH,LH,E2 and T between the large follicle group,the medium follicle group and the small follicle group(P>0.05).The fertilization rate of two pronuclear in large follicle group was higher than that in small follicle group and medium follicle group,the difference was statistically significant(P<0.05).The available embryo rate of large follicle group was higher than that of small follicle group and medium follicle group,the difference was statistically significant(P<0.05).Conclusion The concentrations of FSH,LH,T in follicular fluid were closely related to oocyte maturity.The oocyte development potential of MⅡfollicles obtained from the follicles≥18 mm was better than that from the follicles<18 mm.
