FFR对缺血性心肌病患者PCI术后长期预后的预测价值 被引量:2

The value of FFR in predicting long-term prognosis of patients with ischemic cardiomyopathy after PCI

摘要 目的探究血流储备分数(FFR)对缺血性心肌病患者经皮冠状动脉介入(PCI)术后长期预后的预测价值。方法选择248例行PCI缺血性心肌病患者作为研究对象,根据FFR的正常值(0.75)将患者分为高FFR(≥0.75)组和低FFR(<0.75)组。研究终点为死亡事件,研究指标为心血管不良事件(MACE)。结果两组性别、肌酐、高密度脂蛋白胆固醇(HDL-C)、直径狭窄(DS)、血管病变1支和3支方面存在显著差异(P<0.05);而低密度脂蛋白胆固醇(LDL-C)、尿酸、年龄、血红蛋白、吸烟史、白细胞计数、心率、高血压史、体重指数(BMI)、三酰甘油(TG)、左心室射血分数(LVEF)、血管病变2支、糖尿病、用药类型、红细胞分布宽度(RDW)无显著性差异(P>0.05);低FFR组出院后MACE发生率、住院死亡和出院死亡率均显著高于高FFR组(均P<0.05);但两组发生脑卒中、靶血管重建、再发心肌梗死无统计学差异(P>0.05);采用单因素Cox回归分析缺血性心肌病患者的死亡事件,结果心率、血管病变数、男性、低FFR、HDL-C、TG、肌酐、总胆固醇、LVEF与死亡事件具有显著相关关系(均P<0.05)。多因素Cox回归分析,结果患者死亡的独立影响因素为男性(HR=1.02,95%CI:1.01~1.05,P=0.001)、血管病变数(HR=1.09,95%CI:1.06~1.31,P=0.000)、低FFR(HR=2.23,95%CI:1.18~4.35,P=0.019)、LVEF(HR=0.91,95%CI:0.86~0.97,P=0.003)。生存分析显示,低FFR组累计生存率显著低于高FFR组(Log Rank=7.81,P=0.007)。低FFR组累计死亡率明显高于高FFR组(40.00%vs 18.09%,χ^(2)=11.647,P=0.0006)。结论低FFR是缺血性心肌病患者PCI术后长期死亡的危险因素,对于患者PCI术后的危险分层具有重要的预测意义。
