张玉珍教授运用膏方治疗卵巢早衰患者成功受孕验案举隅

Successful pregnancy cases report of treating premature ovarian failure with paste prescription by Professor ZHANG Yuzhen

张玉珍教授从事中医妇科临床、教学工作50载,对妇科疾病的诊疗有着丰富的经验,是国家“十一五”“十二五”全国统编教材《中医妇科学》的主编,她在妇科疑难病不孕症及卵巢早衰的治疗上经验丰富,特别是近年来运用膏方治疗早衰患者助孕,屡见奇功,其中,张教授使用最多的是在其经验方滋癸益经汤基础上加减熬制而成的膏方滋癸益经膏。滋癸益经汤出自《景岳全书》中的三个经典方:归肾丸、毓麟珠、大补元煎,是三个古方和合化裁而成的,全方兼顾阴阳气血,滋补力强,作用全面,补虚扶正,非常适合气血精亏虚、精血虚衰、未老先衰的以虚为本的早衰患者服用。妇女有经带胎产乳,以血为用,以肾气主导,常有阴血亏虚、肾精不足之症,膏方滋补力强,治疗妇科慢性病有独特的优势。文章所录验案,彭女士,30岁,发现卵巢早衰不孕4年余,常见烦躁抑郁,烘热汗出,阴道干涩,性欲下降,腰酸。张教授运用中药加膏方为其治疗,时隔仅1年,彭女士便由最初的烦躁抑郁的严重围绝经期症状转化为成功受孕,这也是张教授众多早衰治疗后成功受孕的验案之一,体现了张教授以中西融合,滋养天癸为主要思路的临证特点,其经验值得我辈研习。

Professor ZHANG Yuzhen has been engaged in the clinical and teaching work of TCM gynecology for 50 years,and has rich experience in the diagnosis and treatment of gynecological diseases.She is the chief editor of the national textbook“Gynecology of Traditional Chinese Medicine”during the 11th five-year plan and 12th five-year plan.She has rich experience in the treatment of difficult gynecological diseases such as infertility and premature ovarian failure,especially in recent years,she has often achieved remarkable achievements in the use of paste to treat patients with premature ovarian failure.The prescription most frequently used is Zigui Yijing decoction(滋癸益经汤)which is on the basis of modified Zigui Yijing decoction.Zigui Yijing decoction comes from the three classic prescriptions in Jingyue Quanshu(《景岳全书》):Guishen pill(归肾丸),Yulinzhu(毓麟珠)and Da Buyuan decoction(大补元煎).The whole prescription takes into account Yin(阴)and Yang(阳),Qi(气)and blood,and has a strong tonic force and comprehensive effect.It is very suitable for patients with deficiency of Qi and blood,deficiency of essence and blood,and premature senility based on deficiency.The woman has the menstruation to take the fetus and to produce milk,in which the blood is of significance and the kidney Qi dominates,and women often suffer from deficiency of Yin and blood and deficiency of kidney essence.The paste prescription has a strong tonic force.It has a unique advantage in the treatment of chronic gynecological diseases.In the case recorded in this paper,Ms.PENG,30 years old,has suffered from premature ovarian failure and infertility for more than 4 years,and shows symptoms of anxiety and depression,hot sweating,vaginal dryness,decreased libido,lumbar acid.Professor ZHANG applies paste prescription to treat her.After only one year,Ms.PENG turns from the initial symptoms of severe perimenopausal depression to successful pregnancy.This is also one of the successful pregnancy cases of Professor ZHANG’s many premature senile treatment,which reflects the characteristics of Professor ZHANG’s academic thought of integrating TCM and Western medicine and nourishing Tiangui(天癸),which is worth of studying.
