没食子水提取物对牙根面刮治保护作用的研究

摘要 目的 探讨没食子水提取物对细菌在刮治牙本质小管渗透的影响,以及对牙根面刮治的保护作用。方法 选取中国人民解放军空军军医大学空军第九八六医院2020年6月至2021年6月收治患者拔除的正畸前磨牙55颗进行根管预备,分别暴露于链球菌、黏性放线菌或链球菌、牙龈卟啉单胞菌培养,检测细菌的侵袭性;将人牙髓细胞接种到牙髓腔中,暴露于菌群培养,采用实时逆转录聚合酶链式反应和免疫分析法检测白细胞介素8(IL-8)、单核细胞趋化蛋白-1(MCP-1)和基质金属蛋白酶-3(MMP-3)的相对表达水平。结果 根面刮治可显著增加细菌的渗透性(P<0.05),没食子水提取物对刮治根面的保护作用显著(P<0.05);经没食子水提取物和根面刮治处理后,细菌渗透性显著降低(P<0.05)。mRNA检测结果显示,没食子水提取物可显著降低刮治后IL-8,MCP-1,MMP-3 mRNA的相对表达水平(P<0.05);但没食子水提取物与氯己定相比,其相对表达水平的差异无统计学意义(P>0.05)。结论 根面刮治去除牙骨质,会使细菌及其毒素从牙周袋渗透到牙本质小管系统,而没食子水提取物可降低细菌的渗透性,具有保护活髓牙牙髓细胞的潜力。 Objective To investigate the effect of water extract from Galla Turcica on the permeation of bacteria in dentinal tubules,and its protective effect on the root surface after scaling.Methods orthodontic treatment admitted to the No. 986 Hospital of Air Force Medical University from June 2020 to June 2021 were selected for the root canal preparation,and then exposed to the Streptococcus gordonii/Actinomyces oris or Streptococcus gordonii/Porphyromonas gingivalis respectively for culture to detect the permeation of bacteria. The human dental pulp cells were inoculated into the dental pulp cavity and cultured exposing to the flora. The real-time reverse transcription-polymerase chain reaction(rRT-PCR) and immunoassay were used to detect the relative expression levels of interleukin-8(IL-8),monocyte chemoattractant protein-1(MCP-1) and matrix metalloproteinase-3(MMP-3).ResultsRoot scaling could significantly increase the permeability of bacteria(P< 0. 05),and the protective effect of water extract from Galla Turcica on the root surface after scaling was significant(P< 0. 05). After the treatment of water extract from Galla Turcica and root scaling,the permeability of bacteria significantly reduced(P< 0. 05). The results of mRNA detection showed that water extract from Galla Turcica could significantly reduce the relative expression levels of IL-8,MCP-1 and MMP-3 mRNA after scaling(P< 0. 05),but there was no significant difference in the relative expression levels of the above mRNA between the Galla Turcica + scaling group and the chlorhexidine + scaling group(P > 0. 05).Conclusion bacteria and their toxins from periodontal pocket to the dentinal tubule system,while the water extract from Galla Turcica can reduce the permeability of bacteria,which has the potential to protect pulp cells of vital teeth.
