摘要
目的探讨三叶因子2(TFF2)、Raf激酶抑制蛋白(RKIP)及miR-181a在慢性萎缩性胃炎(CAG)患者胃黏膜组织中的表达情况,并对三者间的相关关系进行分析。方法选取2018年3月至2021年3月该院收治的CAG患者115例作为CAG组,并招募同期体检健康者120例作为健康对照组,采用免疫组化法检测TFF2和RKIP的表达水平,采用实时荧光定量聚合酶链反应检测miR-181a的表达水平,比较两组间TFF2、RKIP及miR-181a表达水平,比较不同特征CAG患者TFF2、RKIP及miR-181a表达水平,分析三者间的相关关系,并进一步分析影响CAG发生的危险因素。结果CAG组有饮酒史、吸烟史比例高于健康对照组,差异有统计学意义(P<0.05)。与健康对照组比较,CAG组的TFF2和miR-181a表达水平升高,RKIP表达水平降低,差异有统计学意义(P<0.05)。不同发病部位、幽门螺杆菌(Hp)感染史、胃炎/胃癌家族史、饮食习惯规律情况CAG患者间的TFF2、RKIP和miR-181a表达水平不同,差异有统计学意义(P<0.05)。TFF2表达水平与miR-181a表达水平呈正相关(r=0.345,P=0.045),TFF2、miR-181a表达水平与RKIP表达水平均呈负相关(r=—0.326,P=0.047;r=—0.625,P=0.008)。Logistic回归分析发现,年龄≥50岁、发病部位以胃窦为主、Hp感染、有胃炎/胃癌家族史、饮食不规律均为影响CAG发生的独立危险因素(P<0.05)。结论TFF2、RKIP表达水平和miR-181a表达水平在CAG患者的胃黏膜组织中升高,应对年龄≥50岁、发病部位以胃窦为主、有Hp感染、有胃炎/胃癌家族史、饮食习惯不规律患者加强随访。
Objective To investigate the expression of intestinal trefoil factor 2(TFF2),Raf kinase inhibitor protein(RKIP)and miR-181 a in gastric mucosa tissues of patients with chronic atrophic gastritis(CAG)and analyze the correlations among them.Methods A total of 115 CAG patients admitted to 81 st Group Army Hospital of PLA from March 2018 to March 2021 were selected as the CAG group,and 120 healthy people were recruited as the healthy control group.Immunohistochemistry was used to detect the expression levels of TFF2 and RKIPs.The expression levels of miR-181 a were detected by real-time fluorescence quantitative polymerase chain reaction.The expression levels of TFF2,RKIP and miR-181 a were compared between the two groups.The expression levels of TFF2,RKIP and miR-181 a in CAG patients with different characteristics were compared,and the correlations between the three indicators were analyzed.The risk factors of CAG were further analyzed.Results The proportion of patients with drinking history and smoking history in CAG group were higher than those in healthy control group,the differences were statistically significant(P<0.05).Compared with healthy control group,the expression levels of TFF2 and miR-181 a in CAG group increased,and the expression levels of RKIP decreased,the differences were statistically significant(P<0.05).The expression levels of TFF2,RKIP and miR-181 a were different in CAG patients with different disease sites,Helicobacter pylori(Hp)infection history,gastritis/cancer family history and healthy eating habits(P<0.05).The expression level of TFF2 correlated positively with the expression level of miR-181 a(r=0.345,P=0.045),the expression level of TFF2 and miR-181 a correlated negatively with the expression level of RKIP(r=—0.326,P=0.047;r=—0.625,P=0.008).Logistic regression analysis showed that age≥50 years old,gastritis mainly in gastric antrum,Hp infection,gastritis/cancer family history and irregular diet were independent risk factors for CAG(P<0.05).Conclusion The expression levels of TFF2,RKIP and miR-181 a increase in the gastric mucosa tissues of CAG patients.Follow-up should be strengthened for patients aged≥50 years,with gastritis mainly in the gastric antrum,Hp infection,gastritis/cancer family history and irregular diet.
