通过生物信息学评估CDKN2A的表达对头颈部鳞状细胞癌预后价值

Evaluate the value of CDKN2A expression on the prognosis of head and neck squamous cell carcinoma assessed by bioinformatics

摘要 目的分析细胞周期依赖性激酶抑制基因(CDKN2A)在头颈鳞状细胞癌(HNSCC)中的表达对其预后的影响。方法通过生物信息学,利用从TCGA数据库和Oncomine下载的头颈鳞癌患者mRNA及临床资料,分析CDKN2A基因在多种癌症中的表达情况、以及其在配对和非配对的头颈鳞癌样本中的表达,采用Kaplan-Meier方法和Cox回归分析CDKN2A在HNSCC中的临床特征关系及预后价值。结果对于TCGA数据库下载的数据进行分析,可以得到CDKN2A在33种癌症中的表达情况,发现CDKN2A在膀胱癌、卵巢癌、宫颈癌、胰腺癌及头颈鳞癌中高表达(P<0.001);只在睾丸生殖细胞癌(TGCT)中表达下降(P<0.001);对546例头颈鳞状细胞癌及正常组织进行分析,发现CDKN2A在HNSCC中高表达(P<0.001);在头颈部鳞状细胞癌中CDKN2A的表达与患者的T分期(P=0.006)、性别(P=0.012)、病理分级(P=0.002)、解剖部位(P<0.001)、吸烟(P<0.001)有关;将546例头颈鳞状细胞癌患者分为高、低表达CDKN2A两组,总体生存时间(OS)、疾病特异性生存期(DSS)、无进展生存期(PFI)具有显著差异;ROC分析CDKN2A mRNA表达水平对预测患者有较好敏感性。结论CDKN2A在头颈鳞状细胞癌中高表达,与其临床特征和患者预后情况具有相关性,可以作为预测头颈部鳞状细胞癌不良预后的一种潜在标志物。 Objective To analysis the expression of cell cycle-dependent kinase inhibitory gene(CDKN2A)in head and neck squamous cell carcinoma(HNSCC)and its impact on the prognosis.Methods The expression of CDKN2A gene in various cancers and its expression in paired and unpaired head and neck squamous cell carcinoma samples were analyzed by bioinformatics using the mRNA and clinical data of head and neck squamous cell carcinoma patients downloaded from the TCGA database and Oncomine.Kaplan-Meier method and Cox regression were used to analyze the clinical characteristics of CDKN2A in HNSCC and its prognostic value.Results The expression of CDKN2A in 33 cancers could be obtained through the analysis of the data downloaded from the TCGA database,and CDKN2A was found to be highly expressed in bladder,ovarian,cervical,pancreatic and head and neck squamous carcinomas(P<0.001).The expression of CDKN2A was decreased only in testicular germ cell carcinoma(TGCT)(P<0.001).The tissues of 546 cases of head and neck squamous cell carcinomas and paraneoplastic tissues were analyzed and CDKN2A was found to be highly expressed in HNSCC(P<0.001).CDKN2A expression in head and neck squamous cell carcinoma was associated with patient's T stage(P=0.006),gender(P=0.012),pathological grade(P=0.002),anatomical site(P<0.001),and smoking(P<0.001).The 546 patients with head and neck squamous cell carcinoma were divided into two groups of high and low expression of CDKN2A,and there were significant differences in overall survival time(OS),disease-specific survival time(DSS),and progression-free survival time(PFI).ROC analysis indicated that CDKN2A mRNA expression levels had good sensitivity in predicting the prognosis patients.Conclusions CDKN2A is highly expressed in head and neck squamous cell carcinoma,an it is correlated with its clinical features and prognosis of patients,so that it could be used as a potential marker to predict the poor prognosis of head and neck squamous cell carcinoma.
