体外反搏联合运动治疗对心绞痛患者运动耐量及moesin抗体表达的影响

The influence of external counterpulsation combined with exercise therapy on exercise tolerance and expression of moesin antibody in patients suffered with stable angina pectoris

摘要 目的研究体外反搏联合运动治疗对稳定型心绞痛患者运动耐量及moesin抗体的表达情况。方法选择2017年9月—2019年8月本院心脏康复中心收治的稳定型心绞痛患者103例作为研究对象,根据其治疗方案的不同被纳入到对照组(仅服用冠心病二级预防药物,25例)、运动治疗组(冠心病二级预防药物加运动治疗,29例)及体外反搏组(二级预防药物联合运动治疗及体外反搏治疗,49例)三组。用运动平板实验检测患者治疗前后运动耐量指标(运动总时间、最大负荷量、运动诱发ST段下移0.1 mv时间);治疗10周后采用酶联免疫吸附试验(Enzyme-linked immunosorbent assay,ELISA)检测入选者血清moesin抗体,对其moesin抗体阳性检出率及OD检测值进行分析。结果三组患者的moesin抗体阳性检出率无明显差异(P>0.05),但其OD检测值运动治疗组及体外反搏联合运动治疗组明显低于对照组(P<0.05),且体外反搏联合运动治疗组明显低于运动治疗组(P<0.05);三组患者的运动总时间、最大负荷量、运动诱发ST段下移0.1 mv时间方面存在明显差异(P<0.05),体外反搏联合运动治疗组运动总时间及运动诱发ST段下移0.1 mv时间最长,最大负荷量最大,运动治疗组次之,对照组时间最短,负荷量最小。结论体外反搏联合运动治疗可以显著降低冠心病稳定型心绞痛患者的Moesin抗体表达;体外反搏联合运动治疗可以改善冠心病稳定型心绞痛患者的运动耐量。 Objective To study the effects of external counterpulsation combined with exercise therapy on exercise tolerance and moesin antibody expression in patients with stable angina pectoris.Methods 103 patients suffered with stable angina pectoris those were admitted to the cardiac rehabilitation centre of our hospital during September 2017 and August 2019 were enrolled as study subjects.According to the treatment scheme they were divided into control group(administrated with secondary prevention drugs for coronary heart disease only,n=25)and exercise therapy group(secondary prevention drugs for coronary heart disease plus exercise therapy,n=29)and external counterpulsation group(secondary preventive drugs combined with exercise therapy and external counterpulsation therapy,n=49).We detected the patient's exercise tolerance index before and after treatment by using stress test(total exercise time,maximum load,exercise-induced ST segment downward movement of 0.1 mv time).After 10 weeks of treatment,the serum levels of moesin antibody of the selected candidates was detected by enzyme-linked immunosorbent assay(ELISA),and the positive detection rate of moesin antibody and the detection value of OD were analyzed.Results There was no significant difference in the positive detection rate of moesin antibody among the three groups of patients(P>0.05),but the OD detection value of the exercise treatment group and the external counterpulsation combined exercise treatment group were significantly lower than the control group(P<0.05),and the external counterpulsation group combined exercise therapy group was significantly lower than that of the exercise therapy group(P<0.05).There were significant differences in the total exercise time,maximum load,and the time when exercise induced ST segment to move down 0.1mv among the three groups(P<0.05).The total exercise time and the movement-induced ST segment downward movement of 0.1 mv of the combined exercise treatment group were the longest,and the maximum load was the largest,followed by the exercise treatment group,and the control group had the shortest time and the smallest load.Conclusions External counterpulsation combined with exercise therapy may significantly reduce the expression of Moesin antibodies in patients with stable angina pectoris.External counterpulsation combined with exercise therapy may improve exercise tolerance in patients with stable angina pectoris.
