四海之腧取穴法结合言语训练治疗脑梗死后运动性失语临床观察

Clinical Observation on Sihai Zhishu Acupoint Selection Combined with Speech Training on Motor Aphasia after Cerebral Infarction

目的观察“四海之腧”取穴法结合言语训练治疗脑梗死后运动性失语的临床效果。方法将2015年7月—2017年7月湖南省宁乡市中医院针灸康复科收治的60例脑梗死后运动性失语患者分为治疗组和对照组,每组30例。对照组给予常规针刺治疗结合言语训练,治疗组给予“四海之腧”取穴法针刺结合言语训练治疗,观察2组患者的临床疗效、神经功能缺损评分和言语功能评分。结果治疗组临床总有效率为90.00%(27/30),高于对照组的70.00%(21/30),差异有统计学意义(P<0.05);治疗后,2组患者的神经功能缺损评分较治疗前均明显改善(P<0.01),且治疗组改善情况优于对照组(P<0.01);治疗后,2组患者言语功能(口语表达、听理解、阅读、复述、书写)评分较治疗前均明显改善,差异有统计学意义(P<0.01),且治疗组言语功能各方面改善情况均优于对照组,差异有统计学意义(P<0.05)。结论“四海之腧”取穴法结合言语训练治疗脑卒中后失语患者,可改善患者语言功能、降低神经功能缺损,临床效果显著。

Objective To observe the clinical effect of Sihai Zhishu acupoint selection combined with speech training on motor aphasia after cerebral infarction.Methods A total of 60 patients with motor aphasia after cerebral infarction admitted to Ningxiang Hospital of Traditional Chinese Medicine from July 2015 to July 2017 were divided into treatment group and control group,with 30 cases in each group.The control group was treated with conventional acupuncture combined with speech training,while the treatment group was treated with Sihai Zhishu acupoint selection combined with speech training.The clinical effect,neurological deficit score and speech function score of the two groups were observed.Results The total effective rate of the treatment group was 90.00%(27/30),which was higher than that of the control group(70.00%,21/30),and the difference was statistically significant(P<0.05).After treatment,the neurological deficit scores of the two groups were significantly improved compared with those before treatment(P<0.01),and the improvement of the treatment group was better than the control group(P<0.01).After treatment,the scores of speech function(oral expression,listening comprehension,reading,retelling,writing)of patients in the two groups were significantly improved compared with those before treatment(P<0.01),and the improvement of all aspects of speech function in the treatment group was better than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Sihai Zhishu acupoint selection combined with speech training in the treatment of motor aphasia after cerebral infarction can improve language function,decrease neurological impairment,and has significantly clinical effect.
