移居高海拔年轻男性高压复氧的动态生理效应

摘要 目的:高原缺氧易诱发人体应激反应,高压复氧可以有效补氧,但压力变化存在应激反应的风险。因此,全时程监控人体高压复氧的动态生理变化及其预后效应,是高原环境下科学补氧的关键。方法:29名拉萨地区健康移居者,采用无线遥测技术,探讨单次2 h 0.11 MPa高压氧干预(当地大气压为0.065 MPa),动态监测高压复氧前中后三个阶段的基本生理体征、心率变异性、心脏负荷及其变化,以此探讨高原环境中高压复氧产生的生理效应。结果:高压复氧过程中,随血氧饱和度的增加(90.13±1.52∼97.18±1.04)人体体温显著升高(P<0.001,η^(2)=0.85),左心室做功显著下降(P<0.001,η^(2)=0.54),心排量逐渐下降(P<0.001,η^(2)=0.54),心率变异性的超低频功率逐渐上升(P<0.001,η^(2)=0.69),低频功率在加压(P<0.01)和减压阶段(P<0.001)显著增加,表明高压复氧明显减轻了心脏负担,该影响受到氧舱压力变化的调节,且高压复氧存在明显的预后效应。结论:高原高压复氧能够有效改善人体缺氧,缓解心脏负荷,为高原环境补氧提供了科学参考。 Objective:Hypoxia at high altitude is easy to induce stress response in human body,while high-pressure reoxygenation can effectively replenish oxygen,but pressure changes have the risk of stress response.Therefore,monitoring the dynamic physiological changes and prognostic effects of high pressure reoxygenation is the key to scientific oxygen supplement at high altitude environment.Methods:Using wireless telemetry technology,29 healthy migrants in Lhasa were investigated for a single 2 hour 0.11 MPa(Megapascal)hyperbaric oxygen intervention(local atmospheric pressure was 0.06523 MPa),and the basic physiological signs,heart rate variability,heart load and their changes in the three stages before,during and after hyperbaric reoxygenation were dynamically monitored,so as to explore the physiological effects of hyperbaric reoxygenation in high altitude environment.Results:In the process of hyperbaric reoxygenation,with the increase of blood oxygen saturation(90.13±1.52∼97.18±1.04),human body temperature increases significantly(P<0.001,η^(2)=0.85),left ventricular work decreases significantly(P<0.001,η^(2)=0.54),cardiac output decreases gradually(P<0.001,η^(2)=0.54),the ultra low frequency power of heart rate variability increases gradually(P<0.001,η^(2)=0.69),and the low frequency power increases significantly in the pressurization(P<0.01)and decompression stages(P<0.001),indicating that hyperbaric reoxygenation significantly reduces the burden on the heart,which is regulated by the change of oxygen chamber pressure.Hyperbaric reoxygenation has obvious prognostic effect.Conclusion:Hyperbaric reoxygenation at high altitude can effectively improve human hypoxia,relieve heart load,and provide a scientific reference for oxygen supplement in high altitude environment.
